小脑中脑裂显微解剖及相关临床入路

小脑中脑裂是小脑和中脑之间的裂隙,位置深、裂内及周围结构复杂,该区域的病变是神经外科手术的难点。暴露该区域的手术入路主要包括:①枕部经小脑幕入路②经幕下小脑上入路(包括后正中、旁正中、极外侧三种入路)③经胼胝体后部入路④经颞部侧脑室三角区入路等。各个不同入路有其不同的适应症和优缺点。本文就小脑中脑裂的显微结构解剖及该区域最常用的手术入路展开综述。 The cerebellar midbrain is a fissure between the cerebellum and the midbrain. The location is deep, the structure in and around the fissure is complex, and the lesion in this area is a difficult point in neurosurgery. Surgical approach to expose the region include: ① occipital transurethral approach through the cerebellum ② through the supratentorial cerebellar approach (including posterior median, next to the median, the extreme lateral approach) ③ by the corpus callosum posterior approach ④ by the temporal Department of lateral ventricle triangle approach. Each different approach has its own different indications and advantages and disadvantages. This article reviews the anatomy of the cerebellum in the brain and the most commonly used surgical approaches in the area.
