以往人们认为,导致心肌梗塞和猝死的主要原因是冠状动脉的血栓栓塞和冠状动脉痉挛,血小板只是对促发血栓形成起一定的作用。近些年来的研究表明,血小板除了能够促进血栓形成外,其聚集后释放的TXA_2还可以引起冠状动脉痉挛,尤其是其所形成的微血栓阻塞心肌小血管,造成心肌微循环障碍,对于心肌梗塞致死性心律失常和猝死等有重要意义。而且有人认为,血小板微血栓造成的微循环障碍,甚至比阻塞一支冠状动脉更严重。因为血小板微血栓能够阻塞心肌小动脉的侧支循环网,给侧支循环的建立造成很大困难。本文观察了60例冠心病病人的血小板聚集功能,报告如下。
