目的:观察微创穿刺术治疗高血压脑出血的疗效。方法:对2010年11月至2011年5月广州市红十字会医院神经外科16例高血压基底节脑出血患者,在出血早期采用微创穿刺血肿清除术治疗,观察术后血肿残留、格拉斯哥预后评分(GOS)。结果:微创穿刺术对血肿清除效果显著,术后3 d血肿残留量≤40%9例;术后7 d血肿残留量≤40%者10例。术后28 d进行GOS评定以判定近期疗效,结果显示死亡4例、长期昏迷2例、重度残疾5例、中度残疾5例。结论:高血压性基底节脑出血,早期通过微创穿刺术进行血肿引流清除,安全、有效,能改善预后。
Objective: To observe the curative effect of minimally invasive puncture on hypertensive intracerebral hemorrhage. Methods: From November 2010 to May 2011, 16 patients with hypertensive basal ganglia cerebral hemorrhage in the Department of Neurosurgery, Guangzhou Red Cross Hospital were treated with minimally invasive puncture hematoma in the early stage of hemorrhage. The postoperative residual hematoma, Glasgow prognosis Score (GOS). Results: The effect of minimally invasive puncture on hematoma was significant. Nine cases had residual hematoma less than 40% 3 days after operation and 10 cases had residual hematoma less than 40% 7 days after operation. GOS was assessed 28 days after surgery to determine the short-term efficacy, the results showed that 4 cases died, 2 cases of long-term coma, 5 cases of severe disability and 5 cases of moderate disability. CONCLUSION: Hypertensive basal ganglia intracerebral hemorrhage can be safely and effectively treated by minimally invasive puncture for hematoma drainage. Prognosis can be improved.
