目的探讨无创通气联合雾化吸入早期治疗对重症哮喘患者临床症状及血气指标的影响。方法选择94例重症哮喘患者纳入研究,采用随机数字表法分为观察组和对照组,每组47例,观察组患者接受无创通气联合布地奈德+沙丁胺醇雾化吸入治疗,对照组患者接受布地奈德+沙丁胺醇雾化吸入治疗,比较两组患者临床症状、血气指标及治疗效果。结果观察组总有效率为97.87%,明显高于对照组的87.23%(χ~2=3.859,P<0.05);咳嗽、气促、喘息、哮鸣音消失时间均明显短于对照组(t=6.109~7.172,P<0.05);血氧饱和度(Sa O_2)、血氧分压(Pa O_2)明显高于对照组,二氧化碳分压(Pa CO_2)明显低于对照组(t=5.749~6.912,P<0.05)。结论无创通气联合雾化吸入治疗有助于促进临床症状缓解,改善气道通气功能和肺泡氧合功能,纠正血气指标,提高治疗效果。
Objective To investigate the effect of noninvasive ventilation combined with early inhalation therapy on clinical symptoms and blood gas indexes in patients with severe asthma. Methods Ninety-four patients with severe asthma were enrolled in the study. Randomly divided into observation group and control group, with 47 cases in each group. Patients in observation group received non-invasive ventilation combined with budesonide + salbutamol inhalation. Patients in control group received budesonide Ned + salbutamol inhalation therapy, clinical symptoms, blood gas indexes and therapeutic effects were compared between the two groups. Results The total effective rate in the observation group was 97.87%, which was significantly higher than that in the control group (χ ~ 2 = 3.859, P <0.05). The disappearance time of cough, shortness of breath, wheezing and wheeze were significantly shorter than that of the control group = 6.109 ~ 7.172, P <0.05). The blood oxygen saturation (Sa O_2) and blood oxygen partial pressure (Pa O_2) were significantly higher than that of the control group, and the PaCO_2 was significantly lower than that of the control group (t = 5.749 ~ 6.912, P <0.05). Conclusion Non-invasive ventilation combined with inhalation therapy can help to improve clinical symptoms, improve airway ventilation and alveolar oxygenation, correct blood gas and improve therapeutic effect.
