目的:检测分析慢性牙周炎(Chronic periodontitis,CP)伴维持性血液透析(Maintenance hemodialysis,MHD)患者的牙周状况,以及血清高敏-CRP(hs-CRP)的水平,探讨牙周炎与维持性血液透析之间的相关性。方法:选择全身健康对照组(Health control group,HC组)22例,慢性牙周炎组(CP组)24例,因慢性肾病进行维持性血液透析组(MHD组)25例,慢性牙周炎+慢性肾病进行维持性血液透析组(CP+MHD组)28例。记录一般情况,并检查其牙周状况以及测定血清hs-CRP水平。分析各组之间牙石指数(CI)、牙龈出血指数(BI)、探诊深度(PD)、附着丧失(AL)和血清hs-CRP水平的差异性。结果:CP+MHD组PD、AL、BI、CI均高于MHD组和HC组(P<0.05),且PD与CP组相比,差异有统计学意义(P<0.05);CP+MHD组血清hs-CRP水平均高于其余3组,与MHD组和HC组相比,差异均有统计学意义(P<0.05),CP组血清hs-CRP水平高于MHD组(P<0.05);hs-CRP与各组PD、AL、BI和CI呈正相关,相关系数(r)分别为0.28、0.55、0.33和0.47(P<0.05)。结论:血液透析可以引起牙周炎患者的临床牙周指数增加,尤其是探诊深度;慢性牙周炎可以升高血液透析患者血清hs-CRP水平。
Objective: To detect and analyze the periodontal status of chronic periodontitis (CP) patients with maintenance hemodialysis (MHD) and the level of serum hs-CRP (hs-CRP) in patients with chronic periodontitis (CP) Correlation between hemodialysis. Methods: Twenty-two patients in control group (HC group), 24 patients in chronic periodontitis group (CP group), 25 patients in maintenance hemodialysis group (MHD group) due to chronic nephropathy, chronic periodontitis + Chronic kidney disease in maintenance hemodialysis group (CP + MHD group) 28 cases. Record the general situation, and check the periodontal status and serum hs-CRP levels. The differences of calculus index (CI), gingival bleeding index (BI), probing depth (PD), attachment loss (AL) and serum hs-CRP were analyzed. Results: The levels of PD, AL, BI and CI in CP + MHD group were significantly higher than those in MHD group and HC group (P <0.05), and the difference between PD group and CP group was statistically significant (P <0.05) The levels of hs-CRP in serum were higher than those in the other three groups (P <0.05). The levels of serum hs-CRP in CP group were higher than those in MHD group (P <0.05). There was a positive correlation between hs-CRP and PD, AL, BI and CI in all groups. The correlation coefficients (r) were 0.28,0.55,0.33 and 0.47 respectively (P <0.05). Conclusion: Hemodialysis can cause clinical periodontal index increase in patients with periodontitis, especially exploration depth; chronic periodontitis can increase serum hs-CRP levels in hemodialysis patients.
