目的探讨抗环瓜氨酸肽抗体(抗CCP抗体)和类风湿因子(RF)联合检测在类风湿关节炎(RA)诊断中的应用价值。方法选择RA患者49例,健康体检查者49例,采用美国雅培(i2000SR)全自动发光仪对抗CCP抗体进行检测,采用奥林巴斯AU2700全自动生化分析透射比浊法对RF进行检测。比较抗CCP抗体联合RF检测与两者单独检测的敏感性和特异性。结果抗CCP抗体的敏感性69.4%(34/49)低于RF的敏感性73.5%(36/49),但差异无统计学意义(P>0.05),而抗CCP抗体的特异性85.7%(42/49)高于RF的特异性79.6%(39/49),差异有统计学意义(P<0.05)。抗CCP抗体联合RF检测的敏感性87.8%(43/49)和特异性91.8%(45/49)均高于单独检测抗CCP抗体和RF的敏感性和特异性。结论抗CCP抗体具有较高的特异性,而联合检测抗CCP抗体和RF更能提高对RA的早期诊断,对RA早期诊断具有重要的意义。
Objective To investigate the diagnostic value of anti-cyclic citrullinated peptide antibody (anti-CCP antibody) combined with rheumatoid factor (RF) in the diagnosis of rheumatoid arthritis (RA). Methods 49 cases of RA patients and 49 cases of physical examination were selected. The anti-CCP antibody was detected by using the American Abbott (i2000SR) automatic luminometer. The transmission electron microscope was used to detect RF with Olympus AU2700 automatic biochemical analyzer. To compare the sensitivity and specificity of anti-CCP antibodies in combination with RF and the two alone. Results The sensitivity of anti-CCP was 69.4% (34/49) lower than that of RF (73.5%, 36/49), but the difference was not statistically significant (P> 0.05), but the anti-CCP antibody specificity was 85.7% 42/49) was higher than the RF specificity of 79.6% (39/49), the difference was statistically significant (P <0.05). The sensitivity and specificity of anti-CCP antibody combined with RF detection were 87.8% (43/49) and 91.8% (45/49), respectively, higher than that of anti-CCP antibody and RF alone. Conclusion Anti-CCP antibody has higher specificity, but combined detection of anti-CCP antibody and RF can improve the early diagnosis of RA, which is of great significance for the early diagnosis of RA.
