目的研究比较代谢综合征(metabolic syndrome)和LADA(latent autoimmune diabetes in adults)型糖尿病人群血浆同型半胱氨酸水平的情况,探讨胰岛素作用不足及糖尿病并发症对血浆同型半胱氨酸的影响。方法分析505例入选人群:代谢综合征组118例、LADA型糖尿病组56例、健康组125例和单纯2型糖尿病206例血同型半胱氨酸的特点,观察胰岛素抵抗或缺陷和同型半胱氨酸水平的相关性,对存在不同糖尿病并发症数目的同型半胱氨酸水平进行分析。结果505例总体血浆同型半胱氨酸水平呈正态分布,代谢综合征组和LADA型糖尿病组的C肽水平及胰岛素抵抗的HOMA-IR(改良公式)和血同型半胱氨酸水平密切相关,合并两个以上并发症的人群血同型半胱氨酸水平显著高于合并两个及以下并发症的糖尿病人群(P<0.01)。结论代谢综合征和LADA型糖尿病人群的血同型半胱氨酸水平和胰岛素的浓度及敏感指数有关,较多的糖尿病并发症同时伴有较高的同型半胱氨酸血症。
Objective To study the plasma homocysteine levels in patients with metabolic syndrome and latent autoimmune diabetes in adults and to explore the effect of insulin insufficiency and diabetic complications on plasma homocysteine. Methods 505 patients were selected: 118 patients with metabolic syndrome, 56 patients with LADA diabetes mellitus, 125 healthy subjects and 206 patients with type 2 diabetes mellitus. Results There was a normal distribution of plasma homocysteine level in 505 patients. The levels of C-peptide and insulin resistance HOMA-IR (metabolic syndrome) and blood homocysteine levels in metabolic syndrome group and LADA diabetes group were closely correlated , Blood homocysteine levels were significantly higher in those with more than two complications compared with those with two or fewer complications (P <0.01). Conclusion The plasma homocysteine levels in metabolic syndrome and LADA diabetes mellitus are related to insulin concentration and sensitive index, and more diabetic complications are associated with higher homocysteinemia.
