目的研究鼻咽癌同期放化疗期间体质量下降规律及相关预测因素。方法回顾性收集广州医科大学附属肿瘤医院82例鼻咽癌同期放化疗患者,中位年龄45岁。运用配对t检验分析放化疗期间患者体质量下降情况,采用Spearman′s秩相关分析探讨高体质量下降(HWL)与临床因素的相关性。结果配对t检验分析发现,同期放化疗期间患者体质量呈每周连续下降(均P<0.05),其中在第4周下降数值最大。82例患者中32例判定为HWL(体质量下降>10%)。Spearman′s秩相关分析显示:HWL与年龄>40岁、女性、较高T分期和TNM分期、3D-CRT技术、较高鼻咽和颈淋巴放疗剂量、Ⅱ度以上白细胞下降、血红蛋白下降相关(均P<0.05)。结论鼻咽癌同期放化疗期间患者体质量持续下降,年龄、性别、T分期、TNM分期、放疗技术、鼻咽和颈部淋巴放疗剂量、白细胞下降程度、血红蛋白下降可能是鼻咽癌放化疗期间体质量下降程度的预测因素。
Objective To study the regularity of body mass decline and related predictors of nasopharyngeal carcinoma during concurrent chemoradiotherapy. Methods Retrospectively collected 82 patients with nasopharyngeal carcinoma concurrent radiotherapy and chemotherapy in Affiliated Tumor Hospital of Guangzhou Medical University. The median age was 45 years. Paired t-test was used to analyze the decline of body weight during radiotherapy and chemotherapy. Spearman’s rank correlation analysis was used to explore the correlation between high body weight loss (HWL) and clinical factors. Results Paired t-test analysis showed that during the same period of radiotherapy and chemotherapy, the patient’s body mass decreased continuously every week (all P <0.05), of which the value of descending was the highest in the 4th week. Thirty-two of the 82 patients were found to have HWL (body weight loss> 10%). Spearman’s rank correlation analysis showed that: HWL was associated with age> 40 years old, female, higher T stage and TNM staging, 3D-CRT technique, higher dose of nasopharyngeal and cervical lymphatic radiotherapy, leukopenia of Ⅱ degree and decreased hemoglobin P <0.05). Conclusion The body mass of patients with nasopharyngeal carcinoma during radiotherapy and chemotherapy remains declining. Age, gender, T stage, TNM stage, radiation therapy, lymphatic radiation dose in nasopharynx and neck, degree of leukopenia and hemoglobin decrease may be related to the duration of radiotherapy and chemotherapy of nasopharyngeal carcinoma Predictors of the degree of decline in body mass.
