目的探讨阴道分泌物联合白细胞介素-6(IL-6)、C-反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)检测预测胎膜早破的价值。方法选择2016年1-11月在岳阳市妇幼保健院定期进行产前检查并住院分娩的335例正常单胎孕妇为研究对象,从妊娠32~35周开始进行阴道分泌物检测及细胞因子检测。将阴道分泌物检测结果异常的孕妇随机分为干预治疗组和对照组,比较两组胎膜早破发生率。结果 335例孕妇中,阴道分泌物检测结果异常198例(59.10%),细胞因子检测结果异常61例(18.21%),阴道分泌物检测结果异常与正常的孕妇细胞因子检测结果异常发生率比较,差异有统计学意义(P<0.05)。335例孕妇中,发生胎膜早破63例(18.81%),阴道分泌物检测及细胞因子检测结果均异常的孕妇中胎膜早破发生率显著高于阴道分泌物检测及细胞因子检测结果均正常的孕妇(P<0.05)。干预治疗组和对照组孕妇胎膜早破发生率比较差异有统计学意义(χ~2=6.564,P<0.05)。结论阴道分泌物检测联合细胞因子IL-6、CRP、TNF-α检测对胎膜早破有一定的预测价值,对阴道分泌物检测和/或细胞因子检测结果异常的孕妇应及时给予干预治疗,能有效预防胎膜早破的发生。
