目的 研究白血病患者P27(Kip1)的表达及其与白血病预后的关系。方法 用蛋白印迹法(Western blot)加化学发光法研究23例急性白血病患者骨髓或外周血中白血病细胞的P27(Kip1)蛋白表达。结果 P27(Kip1)总阳性率为78.26%(18/23),但表达强度不一。急性非淋巴细胞白血病(ANLL)阳性率82.35%(14/17),4例急性淋巴细胞白血病(ALL)中2例阳性,1例慢性粒细胞白血病急变期(CML-BC)为阻性,1例淋巴肉瘤白血病为阳性。P27(Kip1)低表达组与高表达组生存率无显著差异。13例已死亡的ANLL中P27(Kip1)高表达组总生存期长于低表达组(秩和检验P=0.05)。所有进行化疗的18例患者达到完全缓解(CR)者P27(Kip1)平均表达强度高于未完全缓解(NR)者,但无统计学意义(秩和检验P>0.05)。诊断时外周血白细胞数、血乳酸脱氢酶水平与P27(Kip1)无相关性。结论 P27(Kip1)的表达与ANLL的预后可能相关,高P27(Kip1)者预后好。
Objective To study the expression of P27 (Kip1) in leukemia patients and its relationship with the prognosis of leukemia. Methods Western blot and chemiluminescence were used to study the expression of P27 (Kip1) protein in bone marrow or peripheral blood of 23 patients with acute leukemia. Results The positive rate of P27 (Kip1) was 78.26% (18/23), but the expression intensity was different. The positive rate of ANLL was 82.35% (14/17) in 2 cases, 2 cases were positive in 4 cases of acute lymphoblastic leukemia (ALL), 1 case was in chronic phase of chronic myeloid leukemia (CML-BC), 1 Cases of lymphosarcoma leukemia positive. P27 (Kip1) low expression group and high expression group no significant difference in survival. The overall survival of 13 dead ANP patients with P27 (Kip1) overexpression was longer than that of the low expression (P <0.05). The mean expression of P27 (Kip1) was significantly higher in patients with complete remission (CR) than those in patients with complete remission (CR), but no statistical significance was found in all 18 chemotherapy patients (rank sum test, P> 0.05). There was no correlation between peripheral blood leukocyte count and serum lactate dehydrogenase level and P27 (Kip1) at diagnosis. Conclusion The expression of P27 (Kip1) may be correlated with the prognosis of ANLL, and the prognosis of patients with high P27 (Kip1) is good.
