目的 探讨阿奇霉素与免疫刺激剂脾多肽联合治疗支原体肺炎的疗效及对细胞免疫水平的影响.方法 选取120例确诊支原体肺炎患儿(2015年6月-2016年1月)作为研究对象,其中60例患儿(观察组)给予基础治疗+阿奇霉素+脾多肽,对照组60例仅给予基础治疗+阿奇霉素,疗程10 ~ 14 d.结果 观察组患儿的发热、咳嗽、喘憋、肺部哕音的消失时间、住院时间均显著低于对照组,差异有统计学意义(均P<0.05);治疗前,两组患儿外周血中CD3+、CD4+、CD8+、CD4+/CD8+测定值差异无统计学意义(均P>0.05);治疗后,观察组患儿CD3+、CD4+、CD4+/CD8+均显著高于对照组,差异有统计学意义(均P<0.05),CD8+水平显著低于对照组,差异有统计学意义(P<0.05).治疗后,观察组痊愈率66.67%、显效率23.33%、好转率10.00%,对照组痊愈率50.00%、显效率25.00%、好转率21.67%、无效3.33%,两组比较差异有统计学意义(Z=2.170,P<0.05).结论 阿奇霉素与免疫刺激剂脾多肽联合治疗支原体肺炎患儿的临床疗效更加显著,可能与改善患儿的免疫水平有关.“,”Objective To explore the curative effect of azithromycin combined with immunostimulant spleen polypeptides in treatment of Mycoplasma pneumoniae pneumonia (MPP) and the influence on cellular immune level.Methods A total of 120 children diagnosed as MPP definitely from June 2015 to January 2016 were selected and divided into two groups,60 children in observation group were treated by basic treatment,azithromycin,and spleen peptides,and 60 children in control group were treated by basic treatment and azithromycin,the treatment was continued for 10-14 days.Results The disappearance time of fever,cough,dyspnea,and pulmonary rales,hospitalization time in observation group was statistically significantly lower than that in control group (P<0.05).Before treatment,there was no statistically significant difference in the levels of CD3 +,CD4+,CD8+,and CD4+/CD8 + ratio in peripheral blood between the two groups (all P>0.05).After treatment,the levels of CD3+,C14+,CD4+/CD8+ ratio in peripheral blood in observation group were statistically significantly higher than those in control group (all P<0.05),while the level of CD8+ in peripheral blood in observation group was statistically significantly lower than that in control group (P<0.05).The cure rate,obvious effective rate,and improvement rate after treatment in observation group were 66.67%,23.33%,and 10.00%,respectively;the cure rate,obvious effective rate,and improvement rate after treatment in control group were 50.00%,25.00%,and 21.67%,respectively.There was statistically significant difference between the two groups (Z =2.170,P<0.05).Conclusion The clinical effect of azithromycin combined with immunostimulant spleen polypeptides in treatment of MPP is significant,which may be related to the improvement of immune level in children.
