目的:探讨细节管理在重症监护室医院感染控制管理中的应用及实施效果,旨在为ICU医院感染控制管理提供借鉴。方法:选取2013年1月~2014年9月ICU实施护理细节管理后的616例为观察组,2011年1月~2012年9月ICU实施护理细节管理前的572例为对照组。对照组予以基础护理和急危重症疾病护理,观察组在此基础上实施控制ICU医院感染的细节管理。比较两组医院感染、吸入性肺炎和死亡等医院感染相关事件的发生率,以及护理质量评分与护理满意度。结果:两组均未发生医院感染爆发或流行,观察组与对照组医院感染发生率分别为15.75%、23.78%(P<0.05)。观察组吸入性肺炎和死亡发生率均低于对照组(P<0.05或P<0.01),护理质量评分及护理满意度均高于对照组(P<0.05)。结论:细节管理在ICU医院感染控制管理中具有较好的效果,不仅能有效降低医院感染、吸入性肺炎及死亡等相关负性事件的发生率,也可提高护理质量,提高护理满意度。 Objective: To explore the application and implementation of detail management in hospital infection control management in intensive care unit (ICU), so as to provide reference for ICU hospital infection control management. Methods: From January 2013 to September 2014, 616 cases of intensive care management (ICU) from January 2013 to September 2014 were selected as observation group and 572 cases from January 2011 to September 2012 before ICU implementation of nursing detail management as control group. The control group was given basic care and emergency critical illness care. On the basis of this, the observation group implemented the detailed management of ICU nosocomial infection control. The incidence of nosocomial infections, aspiration pneumonia and death among nosocomial infections were compared between the two groups, and the scores of quality of care and satisfaction with nursing. Results: No outbreak or epidemic of nosocomial infection occurred in both groups. The incidence of nosocomial infection in the observation group and control group were 15.75% and 23.78%, respectively (P <0.05). The incidence of aspiration pneumonia and death in the observation group were lower than those in the control group (P <0.05 or P <0.01), and the nursing quality score and nursing satisfaction were higher than those in the control group (P <0.05). Conclusion: Detail management has a good effect in ICU hospital infection control management. It can not only effectively reduce the incidence of nosocomial infections, aspiration pneumonia and death-related negative events, but also improve the quality of nursing care and improve nursing satisfaction.
