探讨婴幼儿肺炎及并发心力衰竭时血清心肌酶的变化。方法采用病例对照方式,对41例婴幼儿肺炎并发心衰和20例无合并症的婴幼儿肺炎患者及20例健康婴幼儿血清心肌酶进行检测;同时分析了血清心肌酶与呼吸频率、心率的关系。结果心衰组血清心肌酶LDH、AST、CPK、α-HBD较健康组高(P<0.05),LDH、AST、CPK较肺炎组高(P<0.05);肺炎组血清LDH、AST、α-HDB较健康组高(P<0.05);心衰组心衰恢复期LDH、AST、α-HBD仍较健康组高(P<0.05)。结论婴幼儿肺炎血清心肌酶LDH、AST、α-HBD可增高;血清心肌酶在肺炎过程中如果明显增高,LDH、AST、CPK分别接近250、52、177时,可能出现心衰或MOF
To investigate the changes of serum myocardial enzymes in infantile pneumonia and concurrent heart failure. Methods 41 cases of infantile pneumonia complicated with heart failure, 20 cases of infantile pneumonia without complications and 20 cases of healthy infants were detected by case control method. Serum myocardial enzymes, respiratory rate, heart rate relationship. Results Serum LDH, AST, CPK and α-HBD in patients with heart failure were significantly higher than those in healthy controls (P <0.05), LDH, AST and CPK were higher in patients with pneumonia than those in patients with pneumonia (P <0.05) AST, α-HDB were higher than those in healthy group (P <0.05). LDH, AST and α-HBD in CHF group were still higher than those in healthy group during heart failure recovery (P <0.05). Conclusions Serum myocardial enzymes LDH, AST and α-HBD may be increased in infants with pneumonia. Serum myocardial enzymes may be associated with heart failure or MOF when LDH, AST and CPK are close to 250, 52 and 177, respectively.
