目的比较潜在转化生长因子β结合蛋白-2(LTBP2)、鳞状细胞抗原(SCC-Ag)以及两者联合检测在宫颈鳞癌及癌前病变诊断中的应用价值。方法通过免疫组化法检测LTBP2在宫颈鳞癌、宫颈上皮内瘤变(CIN)和正常子宫颈鳞状上皮组织中的表达。ELISA法检测LTBP2和SCC-Ag在宫颈鳞癌患者、宫颈上皮内瘤变患者和健康人中的血清学水平。结果 LTBP2表达在宫颈鳞癌中显著高于正常宫颈组织和CIN。与健康人相比,可溶性LTBP2水平在宫颈鳞癌组中显著提高。LTBP2的ROC曲线显示药时曲线下面积(AUC)为0.647±0.043,灵敏度和特异性分别为49.2%和72.6%。联合检测LTBP2和SCC-Ag的ROC曲线分析,AUC为0.908±0.022,特异性为93.5%。结论 LTBP2可能参与宫颈鳞癌发展和转移,LTBP2联合SCC-Ag血清学检测能提高宫颈鳞癌诊断的灵敏度和特异度。
Objective To compare the diagnostic value of latent transforming growth factor β binding protein-2 (LTBP2), squamous cell antigen (SCC-Ag) and their combination in the diagnosis of cervical squamous cell carcinoma and precancerous lesions. Methods Immunohistochemistry was used to detect the expression of LTBP2 in cervical squamous cell carcinoma, cervical intraepithelial neoplasia (CIN) and normal cervical squamous epithelium. Serum levels of LTBP2 and SCC-Ag in patients with cervical squamous cell carcinoma, cervical intraepithelial neoplasia and healthy people were detected by ELISA. Results LTBP2 expression in cervical squamous cell carcinoma was significantly higher than normal cervical tissue and CIN. Soluble LTBP2 levels were significantly higher in cervical squamous cell carcinoma than in healthy people. The ROC curve of LTBP2 showed that the area under the curve (AUC) was 0.647 ± 0.043 and the sensitivity and specificity were 49.2% and 72.6%, respectively. ROC curve analysis of combined detection of LTBP2 and SCC-Ag showed AUC of 0.908 ± 0.022 and specificity of 93.5%. Conclusion LTBP2 may be involved in the development and metastasis of cervical squamous cell carcinoma. LTBP2 combined with SCC-Ag serological test may improve the sensitivity and specificity of cervical squamous cell carcinoma.
