目的研究妊娠期高血压疾病发病相关的危险因素,为妊娠期高血压疾病的防治和重点人群的干预提供理论依据。方法选取2013年3月-2015年3月在该院住院并诊断为妊娠期高血压疾病的孕产妇作为病例组,选择同期住院的健康孕产妇作为对照组,运用流行病学病例对照回顾性分析方法分析影响妊娠期高血压疾病发生的影响因素。结果单因素分析比较发现,年龄≥35岁(OR=2.16,95%CI:1.06~2.96)、孕前体质指数(BMI)≥28 kg/m2(OR=1.65,95%CI:1.23~2.62)、流产次数≥2次(OR=1.72,95%CI:1.26~2.71)、高血压家族史(OR=1.86,95%CI:1.20~3.26)和孕次≥3次(OR=1.13,95%CI:0.79~1.94)是妊娠期高血压疾病发生的危险因素;多因素Logistic回归模型分析发现,年龄≥35岁(OR=2.096,95%CI:1.38~3.85)、高血压家族史(OR=1.62,95%CI:1.21~2.31)和孕前BMI≥28 kg/m2(OR=1.77,95%CI:1.32~2.40)为妊娠期高血压疾病发生的危险因素。结论高龄、孕期超重、高血压家族史是妊娠期高血压疾病发生的危险因素,针对重点人群应该加强干预,预防妊娠期高血压疾病的发生。
Objective To study the risk factors associated with the development of hypertensive disorder complicating pregnancy and to provide a theoretical basis for the prevention and treatment of hypertensive disorder complicating pregnancy and the intervention of key population. Methods From March 2013 to March 2015, maternal women hospitalized and diagnosed as hypertensive disorder complicating pregnancy were selected as the case group, and healthy pregnant women in the same period were selected as the control group. The epidemiological case-control retrospective analysis Methods Analysis of influencing factors of hypertensive disorders during pregnancy. Results Univariate analysis showed that BMI ≥28 kg / m2 (OR = 1.65, 95% CI: 1.23-2.62), age ≥35 years (OR = 2.16,95% CI: 1.06-2.96) (OR = 1.72, 95% CI: 1.26-2.71), family history of hypertension (OR = 1.86, 95% CI: 1.20-3.26) and pregnancy times≥3 (OR = 1.13, 95% CI : 0.79-1.94) were risk factors of hypertensive disorder complicating pregnancy. Multivariate logistic regression analysis showed that family history of hypertension (OR = 1.62, 95% CI: 1.38-3.85) , 95% CI: 1.21 ~ 2.31) and pre-pregnancy BMI≥28 kg / m2 (OR = 1.77,95% CI: 1.32 ~ 2.40) were the risk factors for hypertensive disorder complicating pregnancy. Conclusion The elderly, overweight and family history of hypertension are the risk factors of hypertensive disorders during pregnancy. Intensive intervention should be given to the key populations to prevent the occurrence of hypertensive disorders during pregnancy.
