目的对狂犬病暴露后全程接种人二倍体细胞狂犬病疫苗(human diploid cell rabies vaccine, HDCV)人群的不良反应发生率进行Meta分析,为狂犬病疫苗接种策略提供理论依据.方法通过检索中国知网、中国生物医学文献服务系统、维普期刊资源平台、万方数据知识服务平台、PubMed数据库、Google学术数据库、ScienceDi?rect数据库和Cochrane Library数据库中已发表的与HDCV接种不良反应相关的研究文献,检索时限为1970年01月-2018年07月;根据纳入与排除标准进行数据整理,并利用R软件对不良反应发生率进行Meta数据分析.结果合计纳入19篇中英文文献,且所纳入的文献无明显的发表偏倚.纳入文献中接种HDCV的总调查人数为7 239人,全身不良反应的合并发生率为16.63%,局部不良反应的合并发生率为34.07%;国外HDCV接种后全身不良反应发生率为20.76%,局部不良反应发生率为42.97%;国产HDCV接种后全身不良反应发生率为2.47%,局部不良反应发生率为4.22%;两组数据差异有统计学意义(P<0.01).国外HDCV接种不良反应合并发生率为13.44%~29.19%,国产HDCV接种不良反应合并发生率为0.23%~6.98%,两组数据差异有统计学意义(P<0.01).结论国产HDCV不良反应发生率低,具有良好的安全性.“,”Objective We conducted a Meta-analysis on the incidence of adverse drug reactions among people who re?ceived a full immunization with human diploid cell rabies vaccine (HDCV) throughout the world, in order to provide theoretical basis for the development of rabies vaccination regimen. Method Nineteen literatures about studies on the incidence of adverse drug reactions vaccinated with HDCV, from January, 1970 to July, 2018, were collected from the databases including China Knowledge Resource Integrated Database (CNKI), SinoMed, PubMed, VIP Journal Integra?tion Platform, Wanfang Data, Google Scholar, ScienceDirect and Cochrane Library. The collected data were strictly or?ganized according to the inclusion and exclusion criteria, and Meta-analysis on the incidence of adverse drug reactions was conducted by R software. Result A total of 19 English and Chinese studies were included in the analysis, without any obvious bias. 7239 cases vaccinated with HDCV in total had been investigated. The local adverse drug reactions rate was 34.07%, while the systemic adverse drug reactions rate was 16.63%. The systemic and local adverse drug reac?tions rate of foreign HDCV was 20.76% and 42.97% respectively, meanwhile the systemic and local adverse drug reac?tions rate of Chinese HDCV was only 2.47% and 4.22% respectively. There is a significant difference between these two vaccine categories (P<0.01). The combined incidence rate of foreign rabies vaccines was 13.44%-29.19%, and that of the Chinese rabies vaccine was 0.23%-6.98%, which shows significant difference between the two groups (P<0.01). Conclusion The adverse drug reactions rate caused by Chinese HDCV was significantly lower than that of foreign ra?bies vaccines, demonstrating Chinese HDCV has satisfactory safety and tolerance.
