目的探讨BOLD-fMRI评价原发性肝细胞癌(HCC)患者接受TACE治疗前、后的血氧变化特点及其临床应用价值。方法对10例HCC患者(HCC组)和10名健康志愿者(对照组)进行BOLD-fMRI扫描,对HCC组患者于TA-CE术后2~3周内再次行BOLD-fMRI扫描。结果 HCC组和对照组肝实质T2*值分别为(13.90±2.94)ms和(17.94±5.93)ms,差异无统计学意义(P>0.05)。HCC组TACE治疗前、后富血供HCC组织T2*值分别为(31.91±2.07)ms和(7.69±0.75)ms,差异有统计学意义(P<0.01),TACE治疗前、后肿瘤周围肝脏组织T2*值分别为(19.94±3.05)ms和(24.21±1.98)ms,差异有统计学意义(P<0.01)。结论 BOLD-fMRI能有效评测HCC患者TACE治疗前、后非瘤肝脏组织和肿瘤组织内血氧含量的变化特点。
Objective To investigate the characteristics of BOLD-fMRI in the evaluation of blood oxygen changes in patients with primary hepatocellular carcinoma (HCC) before and after TACE treatment and their clinical application value. Methods BOLD-fMRI was performed on 10 HCC patients (HCC group) and 10 healthy volunteers (control group). BOLD-fMRI scans were performed in HCC patients within 2 to 3 weeks after TA-CE. Results The T2* values of liver parenchyma in HCC and control groups were (13.90±2.94) ms and (17.94±5.93) ms, respectively. There was no significant difference (P>0.05). The T2* values of HCC tissue in the HCC group before and after TACE treatment were (31.91±2.07) ms and (7.69±0.75) ms, respectively (P<0.01). The liver around the tumor before and after TACE treatment The tissue T2* values were (19.94±3.05) ms and (24.21±1.98) ms respectively, and the difference was statistically significant (P<0.01). Conclusion BOLD-fMRI can effectively evaluate the changes of blood oxygen content in non-neoplastic liver tissues and tumor tissues before and after TACE treatment in HCC patients.
