目的:探讨使用有创无创序贯机械通气治疗方法治疗慢性阻塞性肺疾病合并严重呼吸衰竭的疗效。方法:慢性阻塞性肺疾病合并呼吸衰竭患者100例,为其提供抗感染和平喘等一系列治疗;并且采用气管插管方式进行有创机械通气。患者出窗后被随机地分为两组,分别设为序贯治疗组和对照组,各50例,序贯治疗组患者站在出窗后立刻拔出插管,采用口鼻面罩双水平气道正压通气;对照组的患者继续进行有创机械通气。分别对序贯治疗组和患者对照组患者总机械通气和住院时间,呼吸机相性肺炎的发生率,治疗的病死率进行比较。结果:与对照组患者相比较,序贯组患者的各项指标较低。结论:有创无创序贯机械通气治疗慢性阻塞性肺疾病合并呼吸衰竭的效果较好,推荐临床使用.
Objective: To explore the use of invasive non-invasive sequential mechanical ventilation treatment of chronic obstructive pulmonary disease with severe respiratory failure. Methods: A total of 100 patients with chronic obstructive pulmonary disease and respiratory failure were provided with a series of anti-infective and anti-asthma treatments. The patients were also given mechanical ventilation by intubation. Patients were randomly divided into two groups after the window, respectively, as the sequential treatment group and the control group, 50 cases each, sequential treatment group patients standing out of the window immediately after pulling out the cannula, with dual masks nasal mask Road positive pressure ventilation; patients in the control group continued invasive mechanical ventilation. The rates of total mechanical ventilation and hospital stay, the incidence of ventilator-phase pneumonia and the mortality of treatment were compared between the sequential treatment group and the control group. Results: Compared with the control group, the indexes in the sequential group were lower. Conclusions: The invasive and noninvasive sequential mechanical ventilation is effective in treating chronic obstructive pulmonary disease combined with respiratory failure. It is recommended for clinical use.
