目的 评估伽玛刀(γ-刀)治疗对功能性垂体腺瘤的治疗效果。方法 对132例功能性垂体腺瘤病人,用1.5T磁共振和γ-plan计算机联网定位,Leksell伽玛刀实施放射外科手术。肿瘤直径4~37mm,平均14mm,处方剂量11~35Gy,平均26.4Gy。结果 本组病例随访35~57个月,获随访118例89.4%,肿瘤消失78例66.1%,缩小32例27.1%,肿瘤增大1例; 激素值恢复正常74例62.7%,较术前下降26例22.0%;临床症状改善108例91.5%,症状加重6例5.1%。 结论 γ-刀是治疗功能性垂体腺瘤安全、有效的一种方法,但要严格掌握适应证,否则γ-刀治疗后有可能加重症状,产生并发症。
Objective To evaluate the therapeutic effect of Gamma Knife (γ-knife) treatment on functional pituitary adenomas. Methods 132 cases of functional pituitary adenomas were treated with 1.5T magnetic resonance imaging and gamma-plan computer networking. Leksell gamma knife was used for radiosurgery. The tumor diameter was 4 to 37 mm with an average of 14 mm, and the prescription dose was 11 to 35 Gy with an average of 26.4 Gy. The results of this group of patients were followed up for 35 to 57 months, 118 cases were followed up 89.4%, 78 cases of tumor disappeared 66.1%, reduced 32 cases of 27.1%, tumor growth in 1 case; hormone The values returned to normal in 74 cases (62.7%), compared with 26 cases (22.0%) before surgery; 108 cases (91.5%) improved in clinical symptoms, and 6 cases (5.1%) worsened. Conclusion Gamma-knife is a safe and effective method for treating functional pituitary adenomas, but it is necessary to strictly grasp the indications. Otherwise, γ-knife treatment may aggravate the symptoms and cause complications.
