我院自1983年6月至1993年12月收治20岁以下青少年甲状腺单发结节35例,6例(17.1％)病理确诊为癌。为引起对本病重视,现将我们对本病的诊疗体会报告如下。 1 临床资料 1.1 一般资料:本组年龄8～15岁7例,16～20岁28例;男12例,女23例。男女比例为1:1.9。 1.2 症状与体征:35例均以颈部肿物就诊,部份有颈部不适感。病程最短仅1天,最长9年,半年以内者24例(68.6％)。体检35例均为甲状腺单发结节,右叶18例,左叶12例,峡部5例。肿瘤最大直径6cm,最小1.5cm。术前临床诊断:除1例在外院已作病理确诊为甲状腺癌外,余34例均诊为良性结节。 1.3 治疗:本组病人均作手术治疗。其中单纯肿瘤挖除15例,腺叶切除12例,峡部切除2例。3例在术中发现为多发结节而作双侧腺叶次全切除。1例在术中发现为癌而作腺叶切除加改良式颈清扫。1例
Our hospital from June 1983 to December 1993 admitted to adolescents under the age of 20 thyroid single nodules in 35 cases, 6 cases (17.1%) pathologically diagnosed as cancer. In order to arouse the attention of this disease, now we report on the diagnosis and treatment of this disease as follows. 1 Clinical data 1.1 General Information: The group of 8 to 15 years old in 7 cases, 16 to 20 years in 28 cases; 12 males and 23 females. The ratio of male to female is 1: 1.9. 1.2 Symptoms and signs: 35 cases were treated with cervical mass, some have neck discomfort. The shortest duration of only 1 day, up to 9 years, within six months in 24 cases (68.6%). All 35 cases were single thyroid nodules, right lobe in 18 cases, left lobe in 12 cases and isthmus in 5 cases. The largest tumor diameter 6cm, the smallest 1.5cm. Preoperative clinical diagnosis: In addition to a case in the hospital for pathological diagnosis of thyroid cancer, the remaining 34 cases were diagnosed as benign nodules. 1.3 treatment: This group of patients were treated surgically. Among them, 15 cases of simple tumor excision, 12 cases of lobectomy and 2 cases of isthmus resection. Three cases were found in the operation of multiple nodules for bilateral subtotal lobectomy. One case was found in the operation of cancer for gland lobectomy plus modified neck dissection. 1 case
