目的探讨合并不同心血管危险因素的原发性高血压(EH)患者血浆纤维蛋白原(FIB)的变化。方法收集1995—2006年福建医科大学附属第一医院心内科EH患者1300例,按合并心血管危险因素情况分为单纯EH组(196例)、EH合并1个危险因素组(442例)、EH合并2个危险因素组(433例)和EH合并3个及以上危险因素组(229例);根据是否合并代谢综合征(MS)将高血压患者分为EH+MS组(496例)和EH+nonMS组(804例),比较FIB的变化。结果单纯EH患者FIB明显低于合并心血管危险因素的各组EH患者[(3.17±0.89)g/L对(3.37±0.93)g/L、(3.48±0.96)g/L和(3.84±1.13)g/L,P<0.05],高血压合并3个及以上危险因素组FIB显著高于合并1个或2个危险因素组(P<0.05)。EH+MS组较EH+nonMS组FIB显著升高[(3.59±1.01)对(3.38±0.98)g/L,P<0.05)]。调整其他影响因素后,1级、2级和3级EH患者间FIB差异无统计学意义(3.41 g/L、3.42 g/L和3.52 g/L),无心血管危险因素的单纯EH患者在调整其他影响因素后1级、2级和3级EH组间FIB分别为3.15 g/L、3.22 g/L和3.26 g/L,差异无统计学意义。结论 FIB与血压无明显相关性,与EH合并的危险因素密切相关。
Objective To investigate the changes of plasma fibrinogen (FIB) in patients with essential hypertension (EH) complicated by different cardiovascular risk factors. Methods One hundred and thirteen patients with EH in Department of Cardiology of the First Affiliated Hospital of Fujian Medical University from 1995 to 2006 were enrolled and divided into simple EH group (196 cases), EH combined with a risk factor group (442 cases), EH Two groups of risk factors (433 cases) and three groups of risk factors (EH + 229) were enrolled in this study. Hypertensive patients were divided into EH + MS group (496 cases) and EH + nonMS group (804 cases), compare FIB changes. Results The FIB of patients with simple EH was significantly lower than that of EH patients [(3.17 ± 0.89) g / L vs (3.37 ± 0.93) g / L, (3.48 ± 0.96) g / L and [3.84 ± 1.13 ) g / L, P <0.05]. The FIB of hypertension group with 3 or more risk factors was significantly higher than that of 1 or 2 risk factors group (P <0.05). The FIB of EH + MS group was significantly higher than that of EH + nonMS group [(3.59 ± 1.01 vs 3.38 ± 0.98) g / L, P <0.05). After adjusting for other factors, there was no significant difference in FIB between grade 1, grade 2, and grade 3 EH patients (3.41 g / L, 3.42 g / L, and 3.52 g / L). EH patients without cardiovascular risk factors were adjusted After the other factors, the FIB of grade 1, grade 2 and grade 3 EH were 3.15 g / L, 3.22 g / L and 3.26 g / L, respectively, with no significant difference. Conclusion There is no significant correlation between FIB and blood pressure, which is closely related to the risk factors of EH.
