[目的]探讨Reg Ⅳ表达与胃癌腹腔转移的相关性。[方法]利用实时荧光定量PCR检测50例胃癌患者腹腔冲洗液中Reg Ⅳ、CA72-4、CK20的表达水平。[结果]病理诊断已明确发生腹腔转移的12例胃癌腹腔冲洗液中Reg IV的相对表达量为34.3(26.2~39.6),显著高于38例未发生腹腔转移的Reg Ⅳ的相对表达量22.7(18.4~33.0)(P=0.017)。Reg Ⅳ表达水平与患者淋巴结转移、淋巴管侵犯、临床分期等密切相关。淋巴结转移和淋巴管侵犯阳性的患者腹腔冲洗液中Reg Ⅳ的表达水平明显升高(均为P=0.024);晚期胃癌(Ⅲ期和Ⅳ期)患者腹腔冲洗液中Reg Ⅳ的表达水平明显高于早期胃癌患者(Ⅰ期和Ⅱ期)(P=0.015)。Reg Ⅳ在胃癌患者中敏感性(100.0%)优于CA72-4(66.7%)和CK20(58.3%)。[结论]Reg Ⅳ的表达与胃癌的腹腔转移密切相关,利用荧光定量PCR检测胃癌患者的腹腔冲洗液中Reg Ⅳ的水平,可以提高对胃癌腹腔转移的预测。
[Objective] To investigate the relationship between Reg Ⅳ expression and abdominal metastasis of gastric cancer. [Method] Real-time fluorescence quantitative PCR was used to detect the expression of Reg Ⅳ, CA72-4 and CK20 in the peritoneal washings of 50 gastric cancer patients. [Results] The relative expression level of Reg IV in peritoneal irrigating fluid of 12 cases with pathological diagnosis of peritoneal metastasis was 34.3 (26.2-39.6), which was significantly higher than that of Reg IV in 38 cases without peritoneal metastasis (22.7% 18.4 ~ 33.0) (P = 0.017). Reg Ⅳ expression levels are closely related to lymph node metastasis, lymphatic invasion, clinical stage and so on. The expression of Reg Ⅳ in peritoneal washings was significantly increased in patients with lymph node metastasis and lymphatic invasion (both P = 0.024). The expression of Reg Ⅳ in peritoneal washings was significantly higher in patients with advanced gastric cancer (stage Ⅲ and Ⅳ) In patients with early gastric cancer (stage I and stage II) (P = 0.015). Reg Ⅳ was superior to CA72-4 (66.7%) and CK20 (58.3%) in gastric cancer patients (100.0%). [Conclusion] The expression of Reg Ⅳ is closely related to the peritoneal metastasis of gastric cancer. The detection of Reg Ⅳ levels in the peritoneal irrigating solution of gastric cancer by fluorescence quantitative PCR can improve the prediction of peritoneal metastasis of gastric cancer.
