目的:探讨颈动脉重建术在晚期头颈部恶性肿瘤侵犯颈动脉治疗中的应用前景。方法:应用聚四氟乙烯(膨体PTFE)人工血管移植术治疗2例声门上型喉癌颈部淋巴结转移侵犯颈总、颈内动脉患者,术中切除颈总、颈内动脉,一期人工血管移植颈动脉重建。结果:2例术后创口Ⅰ期愈合,无颅脑并发症;彩色多普勒检查显示人工血管血流通畅。分别于2年及3年后死亡。结论:人工血管颈动脉重建术治疗累及颈动脉的晚期恶性肿瘤,能完整切除肿瘤并避免因切除颈动脉引起的暂时或永久性中枢神经系统并发症。
Objective: To explore the application of carotid artery reconstruction in the treatment of advanced head and neck cancer with carotid artery invasion. Methods: Two cases of supratentorial laryngeal carcinoma cervical lymph node metastasis invading the neck and internal carotid artery were treated with polytetrafluoroethylene (expanded PTFE) vascular grafts. The total and internal carotid arteries were removed during the operation. Artificial vascular grafts for carotid artery reconstruction. Results: Two cases of wound healed in the first period without craniocerebral complication. The color Doppler examination showed that the artificial blood flow was smooth. Died in two years and three years respectively. CONCLUSION: Artificial vascular carotid artery reconstruction can treat advanced malignancies involving the carotid artery and can completely excise the tumors and avoid temporary or permanent central nervous system complications caused by the removal of the carotid artery.
