目的研究分析化学发光法与酶联免疫吸附测定法在乙肝两对半检测中的临床应用价值。方法选择西平县人民医院2016年2月—2017年2月收治的乙肝患者500例,抽取血样,分别应用化学发光法和酶联免疫吸附法进行乙肝表面抗原(HBs Ag)、表面抗体(HBs Ab),e抗原(HBe Ag)、e抗体(HBe Ab)、核心抗体(HBc Ab)检测,观察比较两种测定方法的敏感性、特异性和准确性。结果化学发光免疫法对HBs Ag、HBe Ag、HBe Ab的检出率均高于酶联免疫吸附试验检测法,具有统计学意义(P<0.05);在65例经化学发光免疫法检出的低水平(0.06~3.00 IU/ml)阳性标本中,酶联免疫吸附试验检测法检出率为46例(70.8%)(P<0.05),差异具有统计学意义;酶联免疫吸附试验检测法的最低检出浓度为0.14 IU/ml,显著高于化学发光免疫法的0.03 IU/ml,差异具有统计学意义(P<0.05);酶联免疫吸附试验检测法的批次内、批次间变异系数(精密度)分别为3%~5%、5%~10%,显著高于化学发光免疫法的<3%和<5%,差异具有统计学意义(P<0.05)。结论化学发光法具有敏感度强、特异性好、重复性高、操作简便、检出率高等特点,且其对HBs Ag低水平的检出率较为理想,可有效避免误诊或者漏诊事件的发生。
Objective To study the clinical value of chemiluminescence and enzyme-linked immunosorbent assay in the detection of hepatitis B two and a half. Conclusion The chemiluminescence method has the characteristics of high sensitivity, good specificity, high repeatability, simple operation and high detection rate. The detection rate of HBs Ag is ideal, which can effectively avoid the misdiagnosis or missed diagnosis.
