目的探讨2型糖尿病肾病(DN)患者血清同型半胱氨酸(Hcy)检测值及意义。方法对2012年1月-2014年12月在河北区妇女儿童保健中心住院的137例糖尿病患者用速率法测定同型半胱氨酸,用免疫比浊法测定尿微量白蛋白。根据糖尿病患者尿微量白蛋白含量将其分为三组,DM组有30例单纯糖尿病(malb<20mg/L)患者,DN1组有57例早期DN(malb20~200mg/L)患者,DN2组有50例临床DN(malb>200mg/L)患者。另选68例体检健康者作为对照组。采用速率法分别测定各组血清Hcy水平。结果 DN1组、DN2组血清中的上述指标均明显高于对照组及DM组,且随DN的加重而增高;DN2组上述各指标水平均明显高于DN1组差异有统计学意义(P<0.05)。结论 Hcy是2型DN病理生理过程中重要的炎症介质,在2型DN发生、发展过程中可能起重要作用;检测其水平有助于DN病情预后判断。
Objective To investigate the value of serum homocysteine (Hcy) in type 2 diabetic nephropathy (DN) and its significance. Methods 137 patients with diabetes who were hospitalized in Hebei District Women and Children’s Health Center from January 2012 to December 2014 were tested for homocysteine by the rate method and urine microalbumin by immunoturbidimetry. According to the level of urine microalbuminuria in diabetic patients, they were divided into three groups. DM group had 30 patients with simple diabetes mellitus (malb <20mg / L), DN1 group had 57 patients with early DN (malb20-200mg / L) 50 cases of clinical DN (malb> 200mg / L) patients. Another 68 cases of healthy people as a control group. The rate of serum Hcy levels were determined by the rate method. Results The above indexes in DN1 group and DN2 group were significantly higher than those in control group and DM group, and increased with the aggravation of DN. The levels of these indexes in DN2 group were significantly higher than those in DN1 group (P <0.05 ). Conclusions Hcy is an important inflammatory mediator in the pathophysiology of type 2 DN. It may play an important role in the occurrence and development of type 2 DN. Detection of its level contributes to the prognosis of DN.
