[目的]分析乳腺癌患者色素上皮衍生因子(PEDF)表达及其与超声征象的相关性。[方法 ]以208例乳腺癌患者为研究对象,记录肿块的超声征象,探测肿块血流情况,用免疫组化方法测得所有石蜡组织标本中PEDF蛋白的表达情况,分析不同血流分级组和不同超声征象组PEDF表达阳性率的差异。[结果]导管上皮增生性疾病中PEDF蛋白阳性表达率(55.00%)显著高于乳腺癌患者(29.81%),差异有统计学意义(P<0.01)。浸润性导管癌、浸润性小叶癌、黏液癌+浸润性筛状癌PEDF蛋白阳性表达率分别为26.86%、47.37%和57.14%,3组间比较,差异有统计学意义(P<0.05)。彩色多普勒血流分级0级、Ⅰ级、Ⅱ级、Ⅲ级PEDF蛋白表达阳性率分别为46.88%、31.82%、31.48%、13.95%,各组间比较,差异有统计学意义(P<0.05)。肿瘤直径≤2cm的病灶中PEDF蛋白表达阳性率(44.63%)显著高于肿瘤直径>2cm者(14.94%),差异有统计学意义(P<0.05)。[结论]PEDF在乳腺癌患者中表达明显下降,浸润性导管癌患者PEDF表达阳性率最低,PEDF表达与超声可见的宏观影像表现具有相关性。
[Objective] To analyze the expression of pigment epithelium-derived factor (PEDF) in breast cancer patients and its correlation with ultrasound signs. [Methods] Totally 208 breast cancer patients were included in this study. Ultrasound signs of lumps were recorded and the blood flow of the lumps was detected. The expression of PEDF protein in all paraffin tissues was detected by immunohistochemistry. Difference of positive rate of PEDF expression in different ultrasound sign groups. [Results] The positive rate of PEDF protein in ductal epithelial hyperplasia (55.00%) was significantly higher than that in breast cancer (29.81%), the difference was statistically significant (P <0.01). The positive rates of PEDF protein in infiltrating ductal carcinoma, invasive lobular carcinoma, mucinous carcinoma and invasive carcinoma were 26.86%, 47.37% and 57.14%, respectively. There was significant difference between the three groups (P <0.05). The positive rates of grade 0, grade 1, grade 2, and grade 3 PEDF were 46.88%, 31.82%, 31.48% and 13.95% respectively in color Doppler blood flow classification. The differences among the groups were statistically significant (P < 0.05). The positive rate of PEDF protein (44.63%) was significantly higher than that of tumor diameter> 2cm (14.94%) in tumor diameter ≤2cm, the difference was statistically significant (P <0.05). [Conclusion] The expression of PEDF is significantly decreased in patients with breast cancer. The positive rate of PEDF is the lowest in patients with invasive ductal carcinoma, and the correlation between PEDF expression and macroscopic ultrasound findings.
