目的:基于磁共振测量,研究青少年单侧颞下颌关节盘不可复性前移位对髁突高度的影响。方法:选择2010年1月—2013年6月就诊并行随访观察的单侧颞下颌关节盘不可复性前移位青少年患者124例,平均年龄16岁,平均随访时间13.6个月。在磁共振片上测量髁突高度、盘长度及盘移位距离,比较健、患侧以及随访前、后的差异。采用SAS 9.13软件包对所得数据进行统计学分析。结果:患侧关节盘移位距离从5.44 mm增大至6.83 mm(P<0.05);患侧关节盘长度从9.06 mm缩短为8.12 mm(P<0.05);健侧髁突高度从26.07 mm增加至26.82 mm(P<0.05);患侧髁突高度从24.22 mm降低为23.81 mm(P<0.05);健、患侧髁突高度差异从1.85 mm扩大为3.00 mm(P<0.05)。结论:在青少年单侧颞下颌关节盘不可复性前移位患者病程中,患侧器质性病变继续进展,可能是单侧关节盘移位患者发生下颌偏斜的主要原因。
OBJECTIVE: To study the effect of unilateral premolar TMJ discoverability on condylar height in adolescents based on magnetic resonance imaging. Methods: A total of 124 adolescent patients with unmodified unilateral TMJ discs were selected from January 2010 to June 2013 in our hospital. The average age was 16 years and the average follow-up time was 13.6 months. The condylar height, disc length and disc displacement distance were measured on magnetic resonance (MRI) slices. The differences between healthy and affected sides and before and after follow-up were measured. SAS 9.13 software package for statistical analysis of the data obtained. Results: The dislocation distance of the affected joint was increased from 5.44 mm to 6.83 mm (P <0.05); the length of the affected joint was shortened from 9.06 mm to 8.12 mm (P <0.05); and the contralateral condylar height increased from 26.07 mm To 26.82 mm (P <0.05). The condylar height of condyles decreased from 24.22 mm to 23.81 mm (P <0.05). The difference of condylar height between healthy and affected sides enlarged from 1.85 mm to 3.00 mm (P <0.05). CONCLUSIONS: In the course of adolescent unilateral premolar TMJ discourse, the pathological changes of the affected side continue to progress, which may be the main reason of mandibular deviation in unilateral disc replacement.
