目的 :探讨喉癌患者术后临床分期 (p TNM)及其外周血 CD5 7+ 细胞水平对其生存率的影响。方法 :对 34例喉癌患者 [T1~ 2 13,T3~ 42 1例 ;声门上癌 8例 ,声门癌 2 3例 ,声门下癌 3例 ;淋巴结转移 (p N+ ) 10例 ,无淋巴结转移 (p N0 ) 2 4例 ;高分化 (G1 ) 11例 ,中分化 (G2 ) 13例 ,低分化 (G3) 10例 ]及 16例健康人 (对照组 )以 SAP法测定单克隆抗体 CD5 7+ 标记的阳性细胞百分率。术后随访 6 0个月 ,观察 p TNM分期、病理分级、肿瘤生长部位和CD5 7+ 细胞水平与患者生存率的关系。结果 :随访第 12个月末 ,T1~ 2 与 T3~ 4,p N0 与 p N+ ,G1 与 G2 ,3,CD5 7+ ≥ 15 %与 CD5 7+ <15 % ,声门上癌与声门癌及声门下癌之间生存率差异显著 (P <0 .0 5 )。第 2 4个月和第 31个月 ,T1~ 2与 T3~ 4的生存率差异无显著性意义 (P >0 .0 5 ) ,其余指标则差异显著 (P <0 .0 5 )。第 48个月末 ,T1~ 2 与 T3~ 4,G1与 G2 ,3的生存率差异亦无显著性意义 (P >0 .0 5 ) ;只有 p N0 与 p N+ 、CD5 7+ ≥ 15 %与 CD5 7+ <15 % ,声门上癌与声带癌和声门下癌比较 ,其生存率差异显著 (P <0 .0 5 )。 CD5 7+ 细胞水平在 p N0 组明显低于 p N+ 组及对照组 (P <0 .0 5 )。分析 p N+ 和 CD5 7+ 细胞≥ 15 %及 p N0 和 CD5 7+ ?
Objective: To investigate the effect of postoperative clinical stage (p TNM) and peripheral blood CD5 7+ cell level in laryngeal cancer patients on its survival rate. Methods: Thirty-four patients with laryngeal cancer [T1 ~ 2 13, T3 ~ 42 1 cases; supraglottic carcinoma 8 cases, glottic carcinoma 23 cases, subglottic carcinoma 3 cases, lymph node metastasis (p N +) 10 cases, Lymph node metastasis (pN0) 24 cases; well-differentiated (G1) in 11 cases, moderately differentiated (G2) in 13 cases, poorly differentiated (G3) in 10 cases] and 16 healthy subjects (control group) CD5 7+ labeled positive cell percentage. The patients were followed up for 60 months. The relationship between p TNM stage, pathological grade, tumor growth site and CD5 7 + cell level and patient survival rate was observed. Results: At the end of the 12th month of follow-up, supratentorial and glottis cancers T1 ~ 2 and T3 ~ 4, pN0 and pN +, G1 and G2, 3, CD5 7+ 15% and CD5 7+ <15% And subglottic cancer were significantly different (P <0.05). There were no significant differences in the survival rates between T1 ~ 2 and T3 ~ 4 (P> 0.05) at the 24th month and the 31st month, while the other indexes were significantly different (P <0.05). At the end of the 48th month, there was no significant difference in survival rates between T1 ~ 2 and T3 ~ 4, G1 and G2, 3 (P> 0.05); only pN0 and pN +, CD5 7+ ≥15% CD5 7 + <15%, supraglottic cancer and vocal cord cancer and supraglottic cancer, the survival rate was significantly different (P <0. 05).
