月经出血评分对月经过多女性缺铁的预测作用

目的:探讨月经出血评分(MBS)对月经过多女性缺铁的预测作用。方法:选择95例月经过多女性为对象,以自行设计的MBS量表评估经量,记录卫生巾用量及经期天数,完成血清铁(SI)及血红蛋白(Hb)等检查。结果:95例月经过多女性中检出缺铁65例,其中铁耗减34例,缺铁性红细胞生成12例,缺铁性贫血19例。Spearman分析及误差限图分析均表明MBS评分与女性缺铁程度显著正相关(P<0.01),Logistic回归示MBS评分是影响月经过多女性缺铁的重要因素(P<0.01)。ROC分析示,当女性MBS评分≥41.25时,提示有较高缺铁风险,有必要作进一步实验室缺铁筛查,此时预测灵敏度、特异度分别达50.77%、86.67%,ROC曲线下面积为0.705(P<0.01)。结论:MBS测评法操作简易,能有效预测月经过多女性的缺铁风险,或在门诊、社区基层、女性缺铁普查及日常自我保健中有一定应用前景。 Objective: To investigate the predictive value of menstrual bleeding score (MBS) on iron deficiency in women with menorrhagia. Methods: Ninety-five cases of menorrhagia women were selected as object. The amount of menstruation and the number of days of menstruation were recorded by self-designed MBS scale. Serum iron (SI) and hemoglobin (Hb) were examined. Results: 65 cases of iron deficiency were detected in 95 cases of menorrhagia, including 34 cases of iron depletion, 12 cases of iron deficiency erythrocytes and 19 cases of iron deficiency anemia. Spearman analysis and error limit analysis showed that MBS score was significantly and positively correlated with female iron deficiency (P <0.01). Logistic regression showed MBS score was an important factor affecting the iron deficiency in menome women (P <0.01). ROC analysis showed that when the female MBS score ≥41.25, it was suggested that there was a high risk of iron deficiency, and further laboratory iron deficiency screening was necessary. At this time, the sensitivity and specificity of prediction were 50.77% and 86.67% respectively. The area under the ROC curve Was 0.705 (P <0.01). Conclusion: The MBS assessment method is simple and can effectively predict the risk of iron deficiency in menome women, or has certain application prospects in clinic, community grass-roots level, female iron deficiency screening and daily self-care.
