为探讨CT、MRI、BAEP对脑干病变诊断的敏感性及BAEP与脑干病损严重程度和预后的关系。我们对32例脑干病变的患者同时进行了CT、MRI及BAEP检查,并对患者进行格拉斯哥昏迷计分(GCS)及神经功能缺损评分(SSS评分)。根据BAEP结果分成三组,在病后3月对患者进行预后评价。结果:脑干病变CT阳性串53.13%,MRl阳性率90.62%,BAEP阳性率84.38%。MRI及BAEP阳性率高于头颅CT,有显著性差异,P<0.05。BAEP的I组及Ⅱ组患者GCS计分高于Ⅲ组。SSS评分低于Ⅲ组。Ⅲ组患者全部预后不良,与I组及Ⅱ组比较有显著性差异,<0.05。结论:对脑干病变MRI和BAEP的阳性率较高,若同时进行MRI和BAEP检查则高达100%;BAEP是判断脑干病变严重程度及预后较为可靠的指标。
To investigate the sensitivity of CT, MRI and BAEP in the diagnosis of brainstem lesions and the relationship between BAEP and the severity and prognosis of brain stem lesions. We performed CT, MRI, and BAEP simultaneously in 32 patients with brain stem lesions. Glasgow coma score (GCS) and neurological deficit score (SSS score) were also recorded. The patients were divided into three groups according to the results of BAEP and the prognosis was evaluated in March after illness. Results: The brain stem lesions CT positive string 53.13%, MR1 positive rate 90.62%, BAEP positive rate 84.38%. MRI and BAEP positive rate higher than the head CT, there was a significant difference, P <0.05. GCEP scores of BAEP group I and group II were higher than that of group III. SSS score lower than the group Ⅲ. All patients in group Ⅲ had poor prognosis, which were significantly different from those in group I and group Ⅱ (P <0.05). Conclusion: The positive rate of MRI and BAEP in brainstem lesions is high, up to 100% if both MRI and BAEP examination are performed. BAEP is a reliable indicator to judge the severity of brain stem disease and prognosis.
