目的 探讨膜型 1-金属蛋白酶在胃癌组织的表达以及与胃癌侵袭和转移的关系。 方法 采用免疫组织化学 SP方法检测了 4 7例胃癌组织 MT1- MMP的表达。采用秩和 T检验进行临床病理资料分析。 结果 MT1- MMP在正常胃组织中几乎不表达 (1/15 ) ,而在胃癌组织中高表达 (42 /47)。进展期胃癌表达 (41/42 )明显高于早期胃癌 (1/5 ) (P<0 .0 1)。肠型胃癌 (2 4 /2 8)和弥漫型胃癌 (12 /12 )均高表达 MT1- MMP,但弥漫型胃癌的表达更显著 (P<0 .0 1)。浆膜侵袭组表达 (32 /33)与非浆膜侵袭组 (10 /14 )的差别有非常显著的意义 (P<0 .0 1)。淋巴结转移组 (31/32 )与非淋巴结转移组 (11/15 )的差别有非常显著的意义 (P<0 .0 1)。MT1- MMP表达定位在胃癌侵袭生长的边缘区 ,其中以瘤细胞为主 ,部分基质细胞也表达。分布形式主要有两种。带状分布主要见于肠型胃癌的粘膜、粘膜下层以及肠型胃癌和弥漫型胃癌的肌层侵袭。弥漫性分布多见于弥漫型胃癌的全层侵袭。 结论 MT1- MMP表达促进胃癌的胃壁侵袭和淋巴结转移 ,可作为判断胃癌恶性表型的有用指标
Objective To investigate the expression of membrane type 1-metalloproteinase (MMP-1) in gastric carcinoma and its relationship with invasion and metastasis of gastric carcinoma. Methods Immunohistochemical SP method was used to detect the expression of MT1-MMP in 47 gastric cancer tissues. Using rank sum T test for clinical and pathological data analysis. Results MT1-MMP was almost not expressed in normal gastric tissues (1/15), while it was highly expressed in gastric cancer tissues (42/47). The expression of advanced gastric cancer (41/42) was significantly higher than that of early gastric cancer (1/5) (P <0.01). The expression of MT1-MMP was high in intestinal type gastric cancer (24/28) and diffuse gastric cancer (12/12), but was more significant in diffuse gastric cancer (P <0.01). There was a significant difference between the serosal invasion group (32/33) and the non-serosal invasion group (10/14) (P <0.01). The difference between lymph node metastasis group (31/32) and non-lymph node metastasis group (11/15) was very significant (P <0.01). The expression of MT1-MMP is located in the marginal zone of gastric cancer invasion and growth, mainly tumor cells, some stromal cells are also expressed. There are two main forms of distribution. Belt distribution is mainly seen in the intestinal mucosa of gastric cancer, submucosa and intestinal type gastric cancer and diffuse gastric muscle invasion. Diffuse distribution more common in diffuse gastric cancer full-thickness invasion. Conclusion The MT1-MMP expression can promote the gastric cancer invasion and lymph node metastasis, which can be used as a useful indicator to judge the malignant phenotype of gastric cancer
