目的探讨多层螺旋CT在评价矽肺肺功能改变中的应用价值。方法56例矽肺病人及10名年龄匹配志愿者进行吸气与呼气2期扫描,扫描后进行多平面重组和Pulmo软件定量分析。所有受检查者均在1周内进行肺功能检查。记录矽肺基本病变,对各征象用多层螺旋CT进行定量评分,并与肺通气功能指标进行比较。结果矽肺组与对照组各征象的CT评分均有显著性差异(网格状阴影除外,P=0.553),单纯型矽肺组与复杂型矽肺组CT评分无显著性差异(大阴影的CT评分除外)。CT气体潴留得分和肺气肿得分与肺功能结果呈显著的负相关,网格状阴影和FEF 75%、FEF 50%、FEV1呈不显著的正相关,但是和FEV1/FVC没有相关关系。矽结节、支气管扩张及大阴影的CT评分与肺功能检查结果之间没有相关性。气体潴留和肺气肿CT评分与小气道阻塞肺功能指标之间呈较显著的相关关系。结论多层螺旋CT对矽肺肺功能损害的评价具有重要价值。
Objective To investigate the value of multislice spiral CT in evaluating the changes of lung function in silicosis. Methods Sixty-six patients with silicosis and 10 age-matched volunteers underwent two phases of inspiration and expiration. After scanning, multiplanar reconstruction and Pulmo software were used for quantitative analysis. All subjects underwent pulmonary function tests within 1 week. The basic pathological changes of silicosis were recorded, and the signs were scored quantitatively by multislice spiral CT and compared with the indexes of pulmonary ventilation. Results There were significant differences in CT scores between silicosis group and control group (except for grid shadows, P = 0.553). There was no significant difference in CT score between silicosis group and complex silicosis group (except for large shadow CT score ). There was a significant negative correlation between CT scores of GC and emphysema scores and pulmonary function results. There was no significant positive correlation between the reticular shadows and FEF 75%, FEF 50% and FEV 1, but no correlation with FEV 1 / FVC. There was no correlation between CT scans for silicone nodules, bronchiectasis, and large shadows and pulmonary function tests. Gas retention and emphysema CT score and small airway obstruction pulmonary function indicators showed a significant correlation between. Conclusion Multi-slice spiral CT has important value in the evaluation of silicosis lung function.
