目的分析麻疹病毒疫苗株[上海(沪)191,Shanghai191;S191]的免疫保护性,为探讨有效预防控制麻疹的措施提供参考。方法用微量中和试验测定不同人群血清对麻疹病毒疫苗株(S191)和浙江省宁波市麻疹病毒流行株(Ningbo2008-06)的中和抗体(Neutralizing Antibody,NA)水平。结果 S191株麻疹减毒活疫苗(Measles Attenuated Live Vaccine,MV)初次免疫(初免)血清,有MV免疫史麻疹患者急性期血清,麻疹患者恢复期血清,应急接种MV前、后血清,对疫苗株的NA几何平均滴度(Geometric MeanTiter,GMT)分别为1∶35.23、1∶4.16、1∶231.74、1∶37.81、1∶61.03;对麻疹病毒流行株的NAGMT分别1∶19.04、1∶2.21、1∶596.62、1∶32.25和1∶67.54。初免血清及有免疫史麻疹患者急性期血清,对疫苗株的NAGMT分别是流行株的1.85倍(t=2.537,P<0.05)和1.88倍(t=-2.696,P<0.05);麻疹患者恢复期血清对流行株的NAGMT是疫苗株的2.57倍(t=-3.054,P<0.05);而应急接种后血清中和疫苗株和流行株的NAGMT差异无统计学意义。小月龄婴儿血清随着月龄增长对疫苗株和流行株的NAGMT均下降,6月龄和8月龄婴儿对流行株的NAGMT分别为1∶2.60和1∶1.24。结论 S191株MV能中和宁波麻疹病毒流行株,只是中和能力略有下降;应急接种能显著增加麻疹NA水平;母传麻疹抗体6月龄后几乎无保护作用;有必要对现行MV免疫策略进行调整。
Objective To analyze the immunoprotective properties of measles virus vaccine strain [Shanghai (Shanghai) 191, Shanghai191; S191] and to provide reference for the effective prevention and control of measles. Methods Neutralizing Antibody (NA) levels in sera of different populations were tested by micro neutralization assay in measles virus vaccine strain (S191) and Ningbo province Ningbo measles virus epidemic strain (Ningbo2008-06). Results The serums of the first immunization (initial immunization) of measles attenuated live vaccines (S191), the serums of measles athemia patients with measles immunization history and convalescent serum of measles patients were vaccinated before and after vaccination The geometric mean titers (GMTs) of strains were 1:35.23,1:4.16,1:231.74,1:37.81,1:61.03, respectively. The NAGMT of the measles virus strains were 1:19.04,1:2.21 , 1:596.62, 1:32.25 and 1:67.54. The serum level of NAGMT in the serum of the immunized patients and the serum of patients with immune measles were 1.85 times (t = 2.537, P <0.05) and 1.88 times (t = -2.696, P <0.05) The NAGMT of convalescent sera was 2.57 times higher than that of the vaccine strains (t = -3.054, P <0.05). However, there was no significant difference in the NAGMT between the vaccine strains and the epidemic strains after vaccination. The serum NAGMT of young and old age babies decreased with the increase of age, and the NAGMT of the 6-month-old and 8-month-old babies were 1:2.60 and 1: 1.24 respectively for the vaccine strains and the epidemic strains. Conclusion The MV of S191 strain can neutralize Ningbo measles virus epidemic strain, but its neutralization ability decreases slightly. Emergency vaccination can significantly increase the level of measles NA. The maternal measles antibody has little protective effect after 6 months of age. It is necessary to evaluate the current MV immunization strategy Adjust.
