目的 :比较不同类型冠心病患者心率变异性的差异 ,探讨心率变异性的频谱分析在缺血性心脏病诊断中的作用。方法 :对经冠状动脉造影证实的冠心病患者进行分组分析 ,比较各组间 RR、PR间期时域、频域差异。频域分析采用短时程心电信号经快速傅立叶转换成频域函数后进行。低频定义为 0 .0 4~ 0 .1 5 Hz,高频定义为 0 .1 5~ 0 .4Hz,并对其功率进行标准化。结果 :心绞痛组、心梗组 RR间期频谱低频成分明显高于正常组 ,而心梗组低频成分明显高于心绞痛组 ,心绞痛组、心梗组高频成分明显低于正常组而心梗组高频成分明显低于心绞痛组 ;心绞痛组、心梗组 PR间期频谱低频成分显著高于正常组而心绞痛组、心梗组无显著差异 ,心绞痛组、心梗组高频成分显著低于正常组而心绞痛组、心梗组无明显差异。结论 :PR、RR间期频谱低频、高频改变与冠脉血管病变密切相关 ,提示心电图短时程频谱分析对诊断缺血性心脏病有重要意义
Objective: To compare the difference of heart rate variability in different types of coronary heart disease patients and to explore the role of spectrum analysis of heart rate variability in the diagnosis of ischemic heart disease. Methods: The patients with coronary heart disease confirmed by coronary angiography were divided into groups, and the differences of RR and PR interval time and frequency in each group were compared. Frequency domain analysis using short-term ECG signals after fast Fourier transform into the frequency domain function. The low frequency is defined as 0 .04 ~ 0 .1 5 Hz, the high frequency is defined as 0 .1 5 ~ 0 .4Hz, and its power is normalized. Results: The low-frequency components of RR interval spectrum in angina pectoris group and MI group were significantly higher than those in normal group, while the low frequency components in MI group were significantly higher than those in angina pectoris group. The high frequency components in angina pectoris group and MI group were significantly lower than those in normal group and MI group High frequency component was significantly lower than that of angina pectoris group; low frequency component of PR interval in angina pectoris group and myocardial infarction group was significantly higher than that in normal group and angina pectoris group, but no significant difference in myocardial infarction group; high frequency component in angina pectoris group and myocardial infarction group was significantly lower than normal group Group and angina pectoris group, myocardial infarction group no significant difference. Conclusions: The changes of low frequency and high frequency of PR, RR interval spectrum are closely related to coronary artery disease, suggesting that short-term ECG analysis of ECG is of great significance in the diagnosis of ischemic heart disease
