目的:探讨伴虹膜粘连的无晶体眼二期人工晶体植入的手术技巧及疗效。方法:回顾分析35例(均为单眼)伴虹膜粘连的无晶体眼行二期后房型人工晶体植入术患者的临床资料。结果:术后随访3~12个月,矫正视力0.1~1.0。影响视力的主要因素为原发疾病所致眼底病变。术中、术后并发症主要有术中虹膜表面、后面出血,术后前房、瞳孔区渗出,角膜水肿,前房、玻璃体少量积血,一过性眼压升高。经3 d~2周治疗均恢复。结论:充分利用黏弹剂轻柔、细致地处理虹膜前后粘连,准确植入高质量的人工晶体,熟练的显微操作及人工晶体缝合技术,使伴虹膜粘连的无晶体眼二期人工晶体植入均能获得满意的疗效。
Objective: To investigate the surgical technique and effect of phacoemulsification with intraocular lens implantation with iris adhesion. Methods: A retrospective analysis of 35 patients (both monocular) with iris adhesion of aphakia eyes underwent second-stage posterior chamber intraocular lens implantation in patients with clinical data. Results: The patients were followed up for 3 to 12 months and the corrected visual acuity was 0.1 ~ 1.0. The main factor affecting visual acuity is the primary disease caused by fundus lesions. Intraoperative and postoperative complications were mainly intraoperative iris surface, behind the hemorrhage, postoperative anterior chamber, pupil area exudation, corneal edema, anterior chamber, a small amount of vitreous hemorrhage, transient elevated intraocular pressure. After 3 d ~ 2 weeks treatment were restored. Conclusion: The full use of viscoelastic soft and meticulous treatment of iris before and after adhesion, accurate implantation of high-quality artificial lens, skilled manipulation and intraocular lens suture technique, with iris adhesion of aphakia second-phase intraocular lens implantation Can get satisfactory results.
