对西北某三甲医院医疗纠纷影响因素的研究

目的通过对西北某三甲医院近年来发生的医疗纠纷案例进行分析,分析得出影响医疗纠纷发生概率的因素。方法运用回顾性分析2003年1月—2013年2月西北某三甲医院医疗纠纷的相关档案共计286例,按照1∶2进行同期同科室不同医生管理下的不同病例进行对照。需要采集病例的基本信息,患者信息和医方信息。并运用χ2检验和Logistic回归模型对有可能产生医疗纠纷的影响因素进行单因素和多因素的分析。结果单因素分析的结果显示,医疗费用、患方文化程度、职业和医保类别、医务人员的医疗服务态度和行为都是影响医疗纠纷发生的因素。多因素Logistic回归分析结果显示,医务人员的服务态度和服务行为是影响医疗纠纷发生独立因素。结论对医院医疗纠纷具有影响的因素很多,其中不但有患者自身的各方面因素的影响,而且还有社会中各因素的影响。医护人员在其中的作用并不是绝对的主导,但也有一定的影响,作为医护人员,一定要从自身出发,尽量减少对医疗纠纷。 Objective To analyze the cases of medical disputes that occurred in a top three hospital in Northwest China in recent years and analyze the factors that influence the probability of occurrence of medical disputes. Methods A total of 286 cases of medical disputes in a top three hospital in Northwest China from January 2003 to February 2013 were retrospectively analyzed. According to 1: 2, different cases under the control of different doctors in different departments were compared. Need to collect the basic information of patients, patient information and medical information. The χ2 test and logistic regression model were used to analyze the influencing factors that may cause medical disputes by univariate and multivariate analysis. Results The results of univariate analysis showed that medical expenses, the educational level of patients, the occupational and health insurance categories, the medical service attitude and behaviors of medical staff were all the factors influencing medical disputes. Multivariate logistic regression analysis showed that medical staff attitude and service behavior were the independent factors influencing medical disputes. Conclusion There are many factors affecting the hospital medical disputes, including not only the patient’s own various aspects of the impact, but also the impact of various factors in society. The role of health care workers in them is not absolutely dominant, but it also has some influence. As medical staff, we must proceed from ourselves to minimize medical disputes.
