目的:探讨门诊部处方点评情况,为更好地实施处方点评制定、促进合理用药。方法:抽取2012年1月~2013年6月的处方12922张,统计不合理处方张数,不合理处方原因,并对不合理处方点评。结果:598张为不合理处方,不合格率为4.6%,其中不规范处方215张(36%),用药不适宜处方383张(64%),错误率较高的是用法、用量;联合用药方面。结论:该门诊部处方存在一定数量的不合理性,药师与医师应积极沟通,指导临床合理用药,为患者提供更好的药学服务。
OBJECTIVE: To discuss the prescription review of outpatient department and to promote the rational use of medication in order to better implement prescription reviews. Methods: 12,922 prescriptions from January 2012 to June 2013 were drawn, the number of unreasonable prescriptions was counted, the reason of unreasonable prescription was given, and the comments of unreasonable prescriptions were reviewed. Results: 598 cases were unreasonable prescriptions, the unqualified rate was 4.6%, of which 215 cases were non-standard prescriptions (36%), 383 cases (64%) were not suitable prescriptions, and the higher the error rate were the usage and dosage; aspect. Conclusion: There are a number of unreasonable prescriptions in this outpatient department. The pharmacists and physicians should actively communicate with each other to guide clinical rational drug use and provide patients with better pharmacy services.
