目的:探讨应用背向散射积分超声组织定征鉴别良恶性乳腺肿瘤的价值。方法:随机选择乳腺纤维腺瘤15例,乳腺痛15例,在仪器设置同等条件下,分别对肿块进行背向散射积分的采样分析,比较良恶性肿瘤背向散射积分值的差别,探讨其在鉴别良恶性乳腺肿瘤中的敏感性、特异性、恶性预测值、良性预测值及准确性。结果:以背向散射积分值(IBS值)≤28.5dB为最佳临界值,其鉴别良恶性乳腺肿瘤的敏感性73.33%,特异性80%,恶性预测值78.57%,良性预测值75%,准确性76.67%;以IBS值≤28.5dB联合二维显像指标,其敏感性、特异性、恶性预测值、良性预测值及准确性提高到80%、86.67%、85.71%、81.25%、83.33%;而将IBS值联合二维显像、彩色多普勒、频谱多普勒指标,则敏感性、特异性、恶性预测值、良性预测值及准确性均可达到93%以上。结论:应用背向散射积分超声组织定征鉴别良恶性乳腺肿瘤有一定价值.尤其与二维显像、彩色多普勒、频谱多普勒指标相结合更有意义。
Objective: To investigate the value of backscatter integrated ultrasound tissue identification in the differentiation of benign and malignant breast tumors. Methods: 15 cases of breast fibroadenoma and 15 cases of mammary gland adenoma were randomly selected. Under the same conditions of instrument settings, the samples were analyzed by integrating backscatter integrals, and the differences of integrated backscatter values of benign and malignant tumors were compared. To identify the sensitivity, specificity, malignant predictive value, benign predictive value and accuracy in benign and malignant breast tumors. RESULTS: The best critical value was backscattered integral (IBS) ≤ 28.5 dB. The sensitivity was 73.33%, specificity 80%, malignant predictive value 78.57% and benign predictive value 75%. Accuracy is 76.67%; with the IBS value ≤ 28.5dB combined with two-dimensional imaging, the sensitivity, specificity, malignant predictive value, benign predictive value and accuracy are improved to 80%, 86.67%, 85.71%, 81.25%, 83.33 %; while the IBS value combined with two-dimensional imaging, color Doppler, spectral Doppler index, the sensitivity, specificity, malignant predictive value, benign predictive value and accuracy can reach more than 93%. Conclusion: The application of backscattered integrated ultrasound to identify the benign and malignant breast tumors has some value. It is especially meaningful to combine with two-dimensional imaging, color Doppler and spectral Doppler.
