目的:评价3种根管充填材料在不同断面的根管密封性能,分析其影响因素。方法:收集新鲜拔除的完整单根管下颌第一前磨牙60颗,随即分为A、B、C 3组,每组20颗牙,分别用氧化锌丁香油糊剂、Vitapex糊剂、AH-Plus糊剂加牙胶尖以侧压法进行根管充填。然后在距根尖狭窄2,4,6,8mm处垂直于牙体长轴切开牙体,扫描电镜观察不同材料与根管壁之间的结合状态并测量根管壁与根管糊剂间微缝隙宽度。结果:扫描电镜下不同根管封闭剂与根管壁均观察到空隙存在,但AH-Plus组可见材料呈均质状分布,与管壁粘结最为紧密。在距根尖2mm处AH-Plus组的微缝隙宽度(2.25±7.63)μm明显低于氧化锌丁香油糊剂(9.56±3.12)μm和Vitapex糊剂(8.81±1.55)μm(P<0.05);在距根尖4,6,8mm处,AH-Plus组与氧化锌丁香油组、Vitapex组的微缝隙宽度无显著差异(P>0.05)。结论:AH-Plus糊剂对于根管充填特别是根尖有很好的密合性,能有效防止根尖微渗漏现象的发生。
OBJECTIVE: To evaluate the root canal sealing performance of three types of root canal filling materials at different sections and analyze the influencing factors. Methods: A total of 60 fresh mandibular first premolars were collected and divided into groups A, B and C, with 20 teeth in each group. They were treated with zinc oxide, clove oil, Vitapex paste, AH- Plus paste plus gutta-percha pressure method for root canal filling. Then, the tooth was cut perpendicular to the long axis of the tooth at 2, 4, 6, and 8 mm from the apical stenosis. Scanning electron microscopy was performed to observe the bonding state between different materials and the root canal wall and measure the relationship between the root canal wall and the root canal paste Micro slit width. Results: Voids were observed in different root canal sealers and root canal walls under scanning electron microscopy. However, the material in AH-Plus group showed homogenous distribution with the most closely adhered to the tube wall. The width of micro-slit (2.25 ± 7.63) μm in AH-Plus group was significantly lower than that of zinc oxide clove oil paste (9.56 ± 3.12) μm and Vitapex paste (8.81 ± 1.55) μm (P <0.05) There was no significant difference (P> 0.05) in the micro-slit width between the AH-Plus group, the zinc oxide and clove oil group and the Vitapex group at 4, 6 and 8 mm away from the root tip. Conclusion: AH-Plus paste has good adhesiveness to root canal filling, especially apical root canal, which can effectively prevent the apical microleakage.
