目的探讨妊娠糖尿病(GDM)规范管理对妊娠结局的影响。方法将2009年2月-2012年7月我院收治的102例GDM患者随机分为观察组与对照组各51例,观察组接受GDM规范管理,对照组仅接受GDM常规管理,对比分析两组患者的管理效果。结果①管理后,观察组GDM相关知识掌握率显著高于对照组,组间差异有统计学意义(P<0.05)。②管理后,观察组空腹血糖、OGTT 1h血糖显著低于对照组,组间差异有统计学意义(P<0.05)。③观察组剖宫产、羊水过多发生率显著低于对照组,组间差异有统计学意义(P<0.05)。两组患者巨大儿、妊娠高血压综合征、胎膜早破、产程延长、阴道助产、产后出血、新生儿高胆红素血症、新生儿呼吸窘迫综合征发生率相比差异无统计学意义(P>0.05)。结论 GDM规范管理可显著提高患者的糖尿病相关知识掌握率,有利于降低患者的血糖水平,显著降低剖宫产、羊水过多发生率,值得临床应用与推广
Objective To investigate the influence of standardized management of gestational diabetes mellitus (GDM) on pregnancy outcome. Methods A total of 102 patients with GDM treated in our hospital from February 2009 to July 2012 were randomly divided into observation group (51 cases) and control group (51 cases). The observation group received GDM standard management while the control group received GDM routine management only. Two groups were compared Patient management effectiveness. Results ① After the management, the mastery rate of GDM in observation group was significantly higher than that in control group, with significant difference between groups (P <0.05). After administration, the fasting blood glucose in the observation group was significantly lower than that in the control group at 1 hour OGTT, the difference was statistically significant (P <0.05). ③ The incidence of cesarean section and amniotic fluid overuse in the observation group was significantly lower than that in the control group, the difference was statistically significant (P <0.05). The incidence of macrosomia, pregnancy-induced hypertension, premature rupture of membranes, prolonged labor, vaginal midwifery, postpartum hemorrhage, neonatal hyperbilirubinemia and neonatal respiratory distress syndrome in the two groups were not statistically different Significance (P> 0.05). Conclusion GDM standard management can significantly improve the patient’s knowledge of diabetes control rate is conducive to reducing the patient’s blood glucose levels, significantly reduce the incidence of cesarean section, amniotic fluid over, worthy of clinical application and promotion
