目的探讨利用吻合器通过直肠下端粘膜环形切除 ,肛垫悬吊治疗脱垂性内痔的可行性和安全性。方法应用该术式治疗 10例Ⅲ、Ⅳ度环形脱垂内痔患者 ,男性 8例 ,女性 2例 ,平均年龄 6 5岁 ,内痔脱垂病史平均 12年 ,其中 6例曾接受硬化剂注射 ,2例曾接受外剥内扎手术。结果 8例患者吻合后内痔立即完全回缩 ,2例于术后第 2天完全回缩。术后诉肛门部疼痛者 3例 ,尿储留 6例 ,无肛门部水肿及出血。术后平均住院 4d ,随访 2周至 2个月。术后 2周开始痔核萎缩 ,无大便失禁和复发。结论直肠下端环形粘膜切除治疗Ⅲ、Ⅳ度环形脱垂性内痔具有疗效显著、术后并发症少、恢复正常生活早、住院时间短等特点 ,远期效果尚待进一步观察 Objective To investigate the feasibility and safety of anastomosis device for the treatment of prolapsed internal hemorrhoids by circular mucosal resection of the lower rectum and suspension of the anal cushion. Methods This procedure was used to treat 10 cases of Ⅲ and Ⅳ degree ring prolapsed internal hemorrhoids. There were 8 males and 2 females, with a mean age of 65 years. The history of internal hemorrhoids prolapse was 12 years. Six of them received sclerotherapy, Cases of extirpation have undergone surgery. Results 8 patients with complete recovery immediately after anastomosis hemorrhoids, 2 patients completely retracted on the 2nd day after operation. Postoperative complaints anal pain in 3 cases, 6 cases of urinary retention, no anal andema and bleeding. The average postoperative hospitalization 4d, followed up for 2 weeks to 2 months. 2 weeks after hemorrhoids atrophy, no incontinence and recurrence. Conclusions The treatment of Ⅲ and Ⅳ degree annular prolapse hemorrhoids with the lower rectum annular mucosa resection has obvious curative effect, less postoperative complications, early return to normal life and shorter hospital stay. The long-term effect needs to be further observed
