目的:探讨正常妊娠及妊娠高血压疾病(HDCP)孕妇胎盘组织凋亡相关基因Survivin、Bcl-2和Caspase-3的表达及其相互关系。方法:应用免疫组化的方法(S-P法)检测101例HDCP(其中轻度子痫前期51例、重度子痫前期50例,分别为轻度组、重度组)及62例正常晚孕(对照组)胎盘组织Survivin、Bcl-2和Caspase-3的表达。结果:①Survivin主要定位于绒毛细胞滋养细胞胞浆内,在重度组表达显著低于轻度组及对照组(P<0.05);②Bcl-2主要定位于合体滋养细胞胞浆内,Bcl-2阳性表达在各组之间无显著性差异(P>0.05)。③Caspase-3主要定位于合体滋养细胞及细胞滋养细胞胞浆内,Caspase-3表达在轻度组、重度组显著高于对照组(P<0.01)。④Survivin与Caspase-3在两组胎盘组织中表达呈负相关(r=-0.208,P=0.008<0.01);Bcl-2与Caspase-3在两组胎盘组织中表达呈负相关(r=-0.199,P=0.011<0.05);Sur-vivin与Bcl-2基因表达无相关性(P>0.05)。结论:在妊娠高血压疾病患者胎盘组织中Survivin基因表达下调、Caspase-3基因表达上调、Bcl-2基因表达无明显变化。妊娠高血压疾病的发病机制可能与Survivin及Caspase-3表达变化有关。
Objective: To investigate the expression of placental apoptosis-related genes Survivin, Bcl-2 and Caspase-3 in normal pregnant women and patients with pregnancy-induced hypertension (HDCP) and their relationship. Methods: Immunohistochemical method (SP method) was used to detect 101 cases of HDCP (including 51 cases of mild preeclampsia, 50 cases of severe preeclampsia, respectively), and 62 cases of normal pregnancy (control ) Placenta Survivin, Bcl-2 and Caspase-3 expression. Results: ①Survivin mainly localized in the cytoplasm of villous cytotrophoblast cells, and the expression of Survivin in severe group was significantly lower than that in mild group and control group (P <0.05). ②Bcl-2 was mainly located in cytoplasm of syncytiotrophoblast, There was no significant difference between the groups (P> 0.05). (3) Caspase-3 mainly located in the cytoplasm of syncytiotrophoblast and cytotrophoblast. Caspase-3 expression was significantly higher in mild group and severe group than in control group (P <0.01). The expression of Caspase-3 and Survivin was negatively correlated with the expression of Caspase-3 in placentas of both groups (r = -0.208, P = 0.008 <0.01); The expression of Caspase-3 and Bcl- , P = 0.011 <0.05). No correlation was found between Survivin and Bcl-2 gene expression (P> 0.05). Conclusion: The expression of Survivin gene, Caspase-3 gene and Bcl-2 gene in placenta of patients with pregnancy-induced hypertension are down-regulated. The pathogenesis of hypertensive disorders of pregnancy may be related to the changes of Survivin and Caspase-3 expression.
