目的 :观察 2型糖尿病患者血清可溶性细胞间粘附分子 -1(sICAM -1)和可溶性E -选择素 (sE -selec tin)水平的变化以及降糖治疗对其产生的影响。方法 :应用酶联免疫吸附法 (ELISA)检测了 2 0例健康对照和 2 7例 2型糖尿病患者降糖治疗前后血清sICAM -1、sE -selectin水平。结果 :⑴ 2型糖尿病患者降糖治疗前血清sICAM -1和sE -selectin水平显著高于健康对照组 (P <0 .0 1,P <0 .0 0 1) ,而且 2型糖尿病患者血清sICAM -1和sE -selectin之间呈显著正相关 (r =0 .5 5 ,P <0 .0 1)。降糖治疗 12周后患者血清sE -selectin水平显著降低 (P <0 .0 1) ,但sICAM -1水平与治疗前比较无变化。结论 :2型糖尿病患者存在着血管局部的炎症反应 ,这可能是糖尿病人发生动脉粥样硬化的机制之一。降糖治疗有助于改善这种动脉粥样硬化病变的炎症反应。
Objective: To observe the changes of serum soluble intercellular adhesion molecule - 1 (sICAM - 1) and soluble e - selectin (sE - selec tin) in type 2 diabetic patients and the effect of hypoglycemic therapy on them. Methods: Serum levels of sICAM-1 and sE-selectin were detected by enzyme-linked immunosorbent assay (ELISA) in 20 healthy controls and 27 patients with type 2 diabetes. Results: (1) Serum levels of sICAM-1 and sE -selectin in patients with type 2 diabetes before and after hypoglycemic treatment were significantly higher than those in healthy controls (P <0.01, P <0.01), and serum sICAM -1 and sE -selectin was positively correlated (r = 0.55, P <0.01). After 12 weeks of hypoglycemic treatment, serum sE -selectin levels were significantly decreased (P <0.01), but there was no change in sICAM-1 levels compared with those before treatment. Conclusion: Local vascular inflammation exists in type 2 diabetic patients, which may be one of the mechanisms of atherosclerosis in diabetic patients. Hypoglycemic treatment helps to improve the inflammatory response to this atherosclerotic lesion.
