目的:研究盐酸米诺环素联合替硝唑片辅助根管治疗牙周-牙髓联合病变的临床疗效。方法:选择2013年8月~2016年2月在我院进行根管治疗的牙周-牙髓联合病变患者180例,随机分为两组,两组均采用常规根管治疗,对照组口服替硝唑片,观察组采用盐酸米诺环素联合替硝唑片。治疗后1周复查探诊深度和探诊出血,并于治疗后1个月观察临床疗效和不良反应。结果:治疗1周后,两组的探诊深度和探诊出血均比治疗前得到明显的改善(P<0.05),且观察组明显优于对照组(P<0.05);治疗1月后,观察组的有效率为91.11%(72/90),明显高于对照组的78.89%(71/90)(P<0.05);其中观察组治疗原发牙周病继发牙髓病的有效率为87.09%(27/31),原发牙髓病继发牙周病为88.89%(24/27),合并性病变为96.88%(31/32),均明显高于对照组的80.00%(24/30),69.23%(18/26),85.29%(29/34)(P<0.05);两组均出现腹痛、恶心、呕吐等轻微不良反应,停药后症状即可消失,无严重不良反应发生。结论:盐酸米诺环素联合替硝唑片能明显改善牙周-牙髓联合病变的临床症状,提高治疗有效率,且不会增加不良反应,值得应用推广。
Objective: To study the clinical efficacy of minocycline combined with tinidazole in the treatment of periodontal-endodontic diseases. Methods: One hundred and eighty patients with periodontal-endodontic disease undergoing root canal therapy in our hospital from August 2013 to February 2016 were randomly divided into two groups. The two groups were treated by conventional root canal therapy. Nitroglycide tablets, the observation group with minocycline hydrochloride combined tinidazole tablets. One week after treatment, the probing depth and probing bleeding were observed, and the clinical efficacy and adverse reactions were observed one month after the treatment. Results: After 1 week of treatment, the exploration depth and exploration bleeding in both groups were significantly improved (P <0.05), and the observation group was significantly better than the control group (P <0.05). After 1 month of treatment, The effective rate of the observation group was 91.11% (72/90), which was significantly higher than that of the control group (78.89%, 71/90) (P <0.05). In the observation group, the effective rate of secondary pulpitis secondary to periodontal disease Was 87.09% (27/31), 88.89% (24/27) and 96.88% (31/32) respectively in the primary pulp disease, which were significantly higher than 80.00% in the control group (P <0.05). There were mild adverse reactions such as abdominal pain, nausea and vomiting in both groups. After stopping the medicine, the symptoms disappeared without serious Adverse reactions occurred. Conclusion: Minocycline combined with tinidazole tablets can significantly improve the clinical symptoms of periodontal-endodontic joint disease and improve the treatment efficiency, and will not increase adverse reactions, it is worth to promote the application.
