内侧型蝶骨嵴脑膜瘤是颅内常见肿瘤,外科手术治疗仍然充满挑战。随着高分辨的CT、MRI和DSA等技术的发展与普及,术前评估更加细致准确。术前肿瘤的分型、手术方案的制定对手术入路的选择、肿瘤的切除、视神经及海绵窦内神经、血管的保护及颅骨的重建具有重要价值。术中导航、经颅多普勒等辅助技术的应用,能提高肿瘤的切除率和安全性。立体定向放射和分次立体定向放射治疗为海绵窦区脑膜瘤、残留脑膜瘤、复发脑膜瘤及视神经旁脑膜瘤提供一种行之有效的方法。针对脑膜瘤的化学治疗也在不断的探索之中。
Medial sphenoid ridge meningioma is a common intracranial tumor, surgical treatment is still full of challenges. With the development and popularization of high-resolution CT, MRI and DSA techniques, the preoperative evaluation is more detailed and accurate. Preoperative tumor classification, the development of surgical options for the choice of surgical approach, tumor resection, optic nerve and cavernous nerve, blood vessel protection and reconstruction of the skull has important value. Intraoperative navigation, transcranial Doppler and other assistive technology applications, can improve tumor resection rate and safety. Stereotactic and fractionated stereotactic radiotherapy provide an effective method for cavernous sinus meningioma, residual meningioma, recurrent meningioma and optic nerve meningioma. Chemotherapy for meningioma is also constantly explored.
