16层螺旋CT冠状动脉成像的临床应用价值

目的评价16层螺旋CT冠状动脉成像的临床应用价值。方法对47例临床怀疑冠心病患者行16层螺旋冠状动脉成像扫描,利用多平面重组(MPR)、最大密度投影重组(MIP)及容积重组(VRT)技术做图像后处理。对其中11例行常规冠脉造影检查,对比分析检查结果。结果47例(611条)患者541条(85.54%)可用于影像学评价,11例冠状动脉造影患者发现中、重度狭窄血管31条,CTA检出中、重度狭窄血管32条,与冠状动脉造影相符27条,CTA的敏感度为87.10%,特异度为95.54%,阳性预测值为84.38%,阴性预测值为96.40%。结论16层螺旋CT冠状动脉成像技术对病变血管狭窄的评价(冠状动脉腔径狭窄≥50%)有一定的应用价值,可作为一种常规筛查方法。 Objective To evaluate the clinical value of 16-slice spiral CT coronary angiography. Methods Totally 47 patients with suspected coronary heart disease underwent 16-slice spiral coronary angiography. The images were processed by multiplanar reconstruction (MPR), maximum density projection recombination (MIP) and volumetric recombination (VRT). Among them, 11 cases underwent routine coronary angiography, comparative analysis of the test results. Results A total of 541 (85.54%) of 47 patients (611) were eligible for imaging evaluation. Eleven patients with moderate or severe stenosis were found in 11 patients undergoing coronary angiography. Thirty moderate or severe stenosis vessels were detected by CTA. Thirty-two patients with coronary artery angiography Consistent with the 27, CTA sensitivity was 87.10%, specificity was 95.54%, the positive predictive value was 84.38%, the negative predictive value was 96.40%. Conclusion The 16-slice spiral CT coronary angiography has a certain value in the evaluation of vascular stenosis (coronary artery stenosis ≥50%), which can be used as a routine screening method.
