目的 :探讨颈动脉体瘤的超声、CT、MRI、血管造影影像学表现及诊断价值。材料与方法 :9例颈动脉体瘤中5例经手术病理证实 ,4例经血管造影证实 ,其中2例为双侧。回顾性分析颈动脉体瘤的各种影像学表现 ,评价其临床价值。结果 :各种检查均可显示肿瘤部位及其形态 ,颈动脉体瘤的B型超声特点为中等偏低实质回声 ,大于3 0cm的肿瘤可见管道结构 ,但肿瘤上缘观察欠清 ,2例行彩色多普勒超声显示瘤内网状血管 ;CT增强显示为富血管肿瘤 ,其中1例肿瘤均匀强化 ,颈内、外动脉密度与瘤体等同难以分辨 ;MRI的特点为瘤内见流空的血管影像 ,明确显示颈内、外动脉分离和紧贴或镶嵌在肿瘤后外缘及前外缘 ,MRA能良好显示肿瘤与颈总动脉及其分叉的关系 ;血管造影除明确诊断和显示供血动脉外 ,2例超选择插管栓塞咽升支血管后肿瘤血供减少约60 %。结论 :B超、CT和MRI对颈动脉体瘤均有一定的特征性诊断价值 ,以MRI最为明确 ,血管造影主要是明确供血动脉、介入治疗和减少术中出血。
Objective: To investigate the imaging findings and diagnostic value of ultrasound, CT, MRI and angiography of carotid body tumors. Materials and Methods: Among the 9 cases of carotid body tumors, 5 were confirmed by operation and pathology, and 4 cases were confirmed by angiography, of which 2 cases were bilateral. Retrospective analysis of various imaging features of carotid body tumors was performed to evaluate its clinical value. Results: All the examinations can show the location of the tumor and its morphology. The B-mode ultrasonography of the carotid body tumor is characterized by a moderately low parenchymal echo, and the tumor structure larger than 30 cm shows a ductal structure, but the upper edge of the tumor is less clearly observed. Color Doppler ultrasound showed intratumor reticular vessels; CT enhancement showed rich vascular tumors, in which one tumor was evenly strengthened, and the density of the internal and external carotid artery was indistinguishable from that of the tumor; MRI was characterized by seeing empty flow within the tumor. Vascular images clearly show that the internal and external carotid arteries are separated and cling to or embedded in the posterolateral and anterior borders of the tumor. MRA can well show the relationship between the tumor and the common carotid artery and its bifurcation; angiography in addition to a clear diagnosis and display of blood supply Outside the arteries, 2 cases of superselective intubation embolization of the pharyngeal cannulated blood vessels reduced tumor blood supply by approximately 60%. Conclusion : B-ultrasonography, CT and MRI all have certain diagnostic value for carotid body tumors. The MRI is the most clear. The angiography is mainly to clear the blood supply artery, interventional treatment and reduce intraoperative blood loss.
