目的探讨多囊卵巢综合征青年患者血清脂联素水平与胰岛素抵抗的关系。方法病例组为38例青年多囊卵巢综合征患者,对照组为27例同龄健康女性,两组根据体质指数分为肥胖组(体质质数≥25kg/m2)和非肥胖组(体质指数<25 kg/m2)。在月经第3~5天晨(多囊卵巢综合征闭经者B超检查无优势卵泡当天)留空腹血清。采用酶联免疫吸附法测定血清脂联素水平,葡萄糖氧化酶法测定血糖水平,化学发光法测定胰岛素水平,根据后两者计算胰岛素敏感指数。结果①多囊卵巢综合征组与对照组空腹血糖水平比较,差异无统计学意义(P>0.05);②多囊卵巢综合征组空腹胰岛素水平明显高于对照组(P<0.01);③多囊卵巢综合征组血清脂联素水平和胰岛素敏感指数明显低于对照组(P<0.01);④多囊卵巢综合征组肥胖、非肥胖者及对照组肥胖、非肥胖者脂联素水平和胰岛素敏感指数依次递增,血胰岛素浓度依次递减,组间比较,差异有统计学意义(P<0.01)。结论多囊卵巢综合征青年患者血清脂联素水平降低,以肥胖者降低更明显,且可能与胰岛素敏感性及胰岛素抵抗相关。
Objective To investigate the relationship between serum adiponectin and insulin resistance in patients with polycystic ovary syndrome. Methods The cases were 38 cases of young women with PCOS and the control group of 27 healthy women of same age. The two groups were divided into obesity group (body mass ≥25kg / m2) and non-obese group (body mass index <25 kg / m2). In the first 3 to 5 days of menstruation (polycystic ovary syndrome amenorrhea B ultrasound examination without dominant follicles the same day) left fasting serum. Serum adiponectin levels were measured by enzyme-linked immunosorbent assay, blood glucose levels by glucose oxidase method, and insulin levels by chemiluminescence method. Insulin sensitivity index was calculated according to the latter two. Results ① There was no significant difference in fasting plasma glucose between PCOS group and control group (P> 0.05); ② fasting insulin level in PCOS group was significantly higher than that in control group (P <0.01); ③ The levels of serum adiponectin and insulin sensitivity index in patients with cystic ovarian syndrome were significantly lower than those in the control group (P <0.01). ④The levels of adiponectin in obese, non-obese and control groups were significantly higher than those in control group Insulin sensitivity index followed by increasing, followed by decreasing blood insulin levels, compared between groups, the difference was statistically significant (P <0.01). Conclusion The level of serum adiponectin in young patients with polycystic ovary syndrome is lower than that of obese people, and may be related to insulin sensitivity and insulin resistance.
