目的研究复发-缓解型多发性硬化症(RRMS)增强T2*加权血管成像(ESWAN)显示的大脑内静脉(ICV)及其主要属支和深髓静脉(DMV)特征。方法 53例RRMS病人和53例正常对照者行常规MRI和ESWAN。将相位图像行最小强度投影获得ESWAN静脉图,观察特征性的血管改变,包括ICV及其主要属支的评分和DMV表现。2位有经验的影像医师分析所有数据。结果病人ICV及其主要属支的平均分值低于对照组。急性病人的平均分值高于稳定病人。此外,48例病程更长病人的DMV变细、变短,5例短病程病人的DMV扩张、延长。穿通静脉在30个活动性病变中清楚显示,在69个非活动性病变中显示不清。尤其是稳定病人的15个非活动病变中亦清楚显示穿通静脉。结论增强T2*WMR血管成像可检出复发-缓解型多发性硬化症病人的大脑静脉特征,可能提供MS发病机制的重要信息。要点①ESWAN为多发性硬化症提出了新见解。②ESWAN静脉成像清楚显示大脑内静脉和深髓静脉。③复发-缓解型MS病人的大脑内静脉呈现异常。④不同病程病人的深髓静脉表现不同。
Objective To investigate the characteristics of intracerebral venous (ICV) and its major branches and deep vein veins (DMV) in patients with recurrent and remitting multiple sclerosis (RRMS) enhanced T2 weighted angiography (ESWAN). Methods 53 cases of RRMS patients and 53 normal controls underwent routine MRI and ESWAN. ESWAN venograms were obtained by minimal intensity projection of the phase image lines and characteristic vascular changes were observed, including the score of ICV and its major branches and the appearance of DMV. Two experienced imaging physicians analyzed all data. Results The average score of ICV and its main branches of patients was lower than that of the control group. The average score for acute patients is higher than for stable patients. In addition, 48 patients with longer duration of DMV thinner, shorter, 5 patients with short duration of DMV dilation and prolongation. Penetration of the veins clearly demonstrated in 30 active lesions, unclear in 69 non-active lesions. In particular, 15 inactive patients with stable disease are also clearly shown through the vein. Conclusions Enhanced T2 * WMR angiography can detect cerebral venous characteristics in patients with relapsing-remitting multiple sclerosis and may provide important information on the pathogenesis of MS.
