目的 探讨对比绝经后骨质疏松患者 DEXA与 QUS测量对骨质疏松脊椎骨折的预测能力。方法 :应用双能 X线骨密度仪 (DEXA,L unar,DPX- IQ)和超声骨强度仪 (QUS,Sunlight Om-nisense,Research)对 4 9例绝经后骨质疏松脊柱骨折患者脊柱、股骨近端和桡骨、胫骨进行测定、比较。结果 4 9例患者 DEXA腰椎 BMD- T值为 - 2 .99± 1.4 3,股骨近端 BMD- T值为 - 1.95± 1.14;而QUS桡骨 SOS- T值为 - 3.12± 0 .79,胫骨 SOS- T值为 - 1.6 7± 1.73。比较发现 :腰椎 BMD对脊柱骨质疏松骨折有较好的提示 ,而超声的桡骨 SOS对脊柱骨质疏松骨折也有敏感的预示能力。结论 骨质疏松不仅有骨量的减少 ,还包括骨结构的破坏 ,严格的骨质疏松诊断及其骨折预测 ,应提倡多部位DEXA和 QUS测定的联合判断。
Objective To investigate the predictive ability of DEXA and QUS in predicting osteoporotic vertebral fractures in postmenopausal osteoporosis patients. Methods: Spinal and femoral fractures of 49 patients with postmenopausal osteoporotic spine fractures were treated with DEXA, L unar, DPX-IQ and QUS (Sunlight Om-nisense, Research) Proximal and radial, tibia were measured, compared. Results The BMD-T of DEXA lumbar spine was - 2.99 ± 1.4 3 in 49 patients, and - 1.95 ± 1.14 in the proximal femur. The value of SOS-T in QUS was -3.12 ± 0.79, and the SOS - The T value is - 1.6 7 ± 1.73. The results showed that BMD of lumbar vertebrae was a good indication of osteoporotic fractures of the spine, while SOS of the radius of the spine was sensitive to predicting spinal osteoporotic fractures. Conclusion Osteoporosis not only reduces the bone mass, but also includes the destruction of the bone structure, the strict diagnosis of osteoporosis and its fracture prediction. The combined judgment of multi-site DEXA and QUS should be advocated.
